In 2013,the proposal of “targeted poverty alleviation” sounded the clarion call for the battle against poverty in the whole country.After several years of comprehensive policies,we can see that the goal to lift all of rural poor people out of poverty under the current standards has been completed,but there are still some difficulties and problems.On August 16,2019,an article by Xi Jinping,General Secretary of the Communist Party of China Central Committee,on poverty relief was published in the the Qiushi Journal this year.The article by Xi calls for efforts to address prominent issues to ensure rural poor people do not have to worry about food and clothing and have access to compulsory education,basic medical services and safe housing.He pointed out that winning the battle against poverty in an all-round way is facing some problems that need to be solved gradually over the long term.Prosperity for all is impossible without health for all.The problems that need to be solved gradually in the long term include the problems of poverty due to illness and returning to poverty due to illness which “cannot be completed in one battle during the poverty alleviation tough-tackling period”,“it is long-term,and will not disappear with announcing the elimination of absolute poverty in 2020”.The General Secretary Xi requested that “there must be an overall arrangement,and conditions must be created to solve it in stages”.Therefore,at the critical moment of the end of the year of decisive battle against poverty alleviation and building a moderately prosperous society in all respects,it is very important to explore the construction of long-term mechanism of preventing poverty and returning to poverty due to illness to ensure an orderly connection with the rural revitalization strategy.This paper selects A Banner of Inner Mongolia Autonomous Region,a typical “old revolutionary base areas,areas inhabited by ethnic minority groups,border areas and poverty-stricken areas”,as the research sample.The analysis finds that A Banner has introduced a series of health poverty alleviation policies and measures in recent years.After the implementation of policies,the numbers of people of poverty due to illness and returning to poverty due to illness have declined and the health problems of the poor in A Banner have been significantly alleviated,but the proportion of number of people who are poverty due to illness and returning to poverty due to illness in the total number of poor people in A Banner has been increasing year by year.This situation has exposed some problems that cannot be ignored which made the construction of long-term mechanism of preventing poverty and returning to poverty due to illness in A Banner is trapped in various dilemmas.After combing and analyzing,this article believes that the current long-term mechanism for preventing poverty and returning to poverty due to illness in A Banner mainly relies on existing health poverty alleviation policies and basic public health policies which lacks long-term specialized mechanism construction.There mainly exists problems from four policy levels of the security layer:resources guarantee layer: “having resources to treat illness” dilemma that is manifested as an unbalanced allocation of medical and health resources;funds guarantee layer:“having funds to treat illness” dilemma that is manifested as fragile funds of the special item governance of poverty due to illness;service guarantee layer:“having ability to provide service for treating illness”dilemma that is manifested as low primary medical and health service capabilities;prevention guarantee layer:“less illness”dilemma that is manifested as which it is difficult to manage the ideological changes of both sides of governance of poverty due to illness.Applying Public Goods Theory,Resource Allocation Theory and Stakeholder Theory,it is found that the root cause of the deep-seated dilemma comes from the unfair allocation of resources caused by the contradiction in the supply of public goods,the excessive financial welfare caused bythe political environment in the minority areas,the service implementation deviation caused by the pressure system,and the synergism and coupling weakened between stakeholders of governance of preventing poverty and returning to poverty due to illness.Then putting forward targeted optimization suggestions,that is constructing a mechanism based on four-dimensional monitoring mechanism: long-term resources guarantee mechanism,long-term financial guarantee mechanism,long-term service guarantee mechanism,long-term preventive guarantee mechanism.It aims at providing the possibility to solve problems of preventing poverty and returning to poverty due to illness and improving health of rural residents by governance to avoid falling into the“illness-poverty-illness” vicious circle pattern.
