Objective:Perianal abscess is an acute suppurative infection of the soft tissue around the anal canal or its surrounding space.Once diagnosed,it should be treated as soon as possible,usually conservative treatment is not good.In order to prevent the delay of the disease,surgical treatment is needed.One-time radical treatment of perianal abscess is the first choice,which can effectively reduce the recurrence rate of abscess and the probability of anal fistula after operation.However,the postoperative wound is larger,the recovery time is longer,and the wound dressing needs to be changed regularly,which increases the pain and burden of the patients.This study aims to find a breakthrough in dressing change after perianal abscess surgery,and explore whether the application of chymotrypsin dressing change can effectively promote the healing of postoperative wound of perianal abscess,and at the same time reduce the pain and burden of patients.Methods:From October 2019 to December 2020,60 patients with perianal abscess in the Department of Colorectal Surgery of Cangzhou People’s Hospital were selected,and all of them met the inclusion and exclusion criteria.The subjects were divided into control group and experimental group according to the order of admission by random number table method.All patients underwent one-time radical resection of perianal abscess after the same preoperative preparation.The control group(n=30)was treated as routine dressing change group,and the experimental group(n=30)was treated as chymotrypsin dressing change group.The dressing change began on the second day after surgery,The dressings in the control group were removed and the saline gauze was used for dressing change,while the chymotrypsin gauze was used for dressing change in the experimental group.The observation indexes of postoperative day 3,7 and 14 were observed and recorded:Including wound granulation tissue growth,wound healing rate,VAS score.Patients were followed up after discharge,and the time of complete wound healing was counted.All data were statistically analyzed using SPSS19.0.Results:1.There was no statistical significance in the wound pain score between the two groups on the 3rd postoperative day(P>0.05).And there was statistical significance on the 7th,14 th and 21 st day(P<0.05).The pain in the experimental group was significantly lighter than that in the control group.2.The comparison of granulation tissue growth between the two groups showed that there was no granulation tissue growth on the third day after surgery,which was of no significance.There was statistical significance on the 7th,14 th and 21 st day(P<0.05),and the growth of granulation tissue in the experimental group was significantly better than that in the control group.3.There was no significant difference in wound healing rate between the two groups on the 3rd postoperative day.There was statistical significance on the 7th,14 th and 21 st day(P<0.05).The wound healing speed of the experimental group was significantly faster than that of the control group.4.The comparison of wound healing time between the two groups showed statistical significance(P<0.05),and the healing time of the experimental group was significantly shorter than that of the control group.Conclusion:The application of chymotrypsin for dressing change after perianal abscess surgery can effectively relieve the pain of the wound,promote the growth of granulation tissue,and greatly shorten the healing time of the wound.The application is safe,simple,and effective.It has a certain application prospect and is worth popularizing.
