| Background:Perianal-abscess is a common disease in the department of anorectal disease.Patients may have local discomfort of redness,swelling,heat and pain,as well as aversion to cold,fever and other discomfort.Patients with perianal abscess will eventually choose surgical treatment when conservative treatment is effective.However,due to the special location of perianal abscess,postoperative wound pain,inflammation and other factors,the quality of life decreased and the wound healed slowly.How to promote postoperative wound healing and reduce pain in patients with perianal abscess is the problem to be solved in this study.Objective:The purpose of this study was to explore the effect of external application of modified Sihuang ointment on postoperative wound pain,swelling,secretion,granulation growth and healing time in patients of the low Perianal-Abscess of Toxic Heat Flourishing Type,and to study the safety of modified Sihuang ointment.Accumulate data for clinical application and promotion in the future.Methods:(1)Safety study:1.Skin irritation test:eight healthy adult rabbits were randomly divided into intact skin group(n=4)and damaged skin group(n=4).The left side of the peeling area is coated with vaseline,and the right side is coated with modified Sihuang ointment.The medicine was given at the same time every day for 7 days.The skin irritation was observed and recorded 1 hour after each removal and before re-application on the second day.After the last application,the stimulus response was observed and recorded at 1 hour,2,3,4,5,6 and 7 days after the subjects were removed.2.Allergic test:24 healthy dolphins were selected and the left back of the dolphins was removed one day before the experiment.The rats were randomly divided into three groups:drug group,matrix group and control group(n=8).Sensitized contact:the drug was applied on the left side of the dolphin on the 1st,7th and 14th day of the experiment as sensitized contact.Stimulating contact:on the 14th day of the last sensitization,the corresponding concentration of drug was applied to the right back of the dolphin as excitation.The allergic reactions of the three groups were observed when the drugs were removed at 0,24,48,72 hours.3.Acute toxicity test:eighteen rabbits meeting the requirements were randomly divided into three groups:empty ointment group(n=6),vaseline group(n=6)and modified sihuang ointment group(n=6).Exfoliation was performed one day before administration.The activity and poisoning of the animals were observed within 14 days after the drug was given twice according to the maximum dose within 24 hours.The body weight of rabbits was monitored before and on the 4th,7th,10th and 14th day after administration.After the end of the experiment,anatomical exploration was performed,and pathological examination was performed if there was any toxic reaction or organ abnormality.(2)Clinical study:40 patients who met the criteria of narcolepsy were randomly divided into treatment group and control group.Both groups were treated with the same "operation" type,and the routine treatment before and after operation was also the same.The treatment group was given external application of modified sihuang ointment during dressing change,and the control group was given external application of hospital preparation Huangqin ointment as control.The quality of life,pain and secretion of the patients on the 1st,3rd,7th,14th and 21st day after operation were observed.and recorded,and the conditions of edema and granulation tissue morphology on the 1st,3rd,7th and 14th day were observed and recorded,and the recovery time of the patients was recorded.Results:(1)Safety test:1.Skin irritation test:after continuous application for 7 days,the skin irritation reaction on both sides of the intact skin group and the damaged skin group was mild.Seven days after the end of the drug,the left skin of the intact skin group responded to skin irritation,the right side had mild stimulation on the first day after the end of the drug,and the remaining 6 days showed mild stimulation on the right side,while in the damaged skin group,there was mild stimulation on the left side and mild stimulation on the right side within 5 days after the end of the drug,and skin irritation on the 6th and 7th day.2.Skin allergy test:the score of allergic reaction of dolphin in modified Sihuang ointment group was 2,allergic reaction.3.Acute toxicity test:there was significant difference in the body weight of the three groups before and after treatment(P>0.05).After the end of the experiment,the rabbits were killed and dissected,and there was no obvious abnormality.(2)Clinical study:1."quality of life" comparison:on the 3rd,7th and 14th day after operation,there was significant difference between the two groups(P<0.05).The quality of life in the test group was better than that in the control group.2.There was significant difference in pain score between the two groups on the 3rd,7th,14th and 21st day after operation(P<0.05).The postoperative wound secretion in the experimental group was better than that in the control group.3.Wound edema:on the 3rd,7th and 14th day after operation,the secretion of the two groups was P<0.05,and the difference was statistically significant.The postoperative swelling of the patients in the experimental group was better than that in the control group.4.Tissue morphology of buds:on the 3rd,7th and 14th day after operation,the P values of the two groups were all at 0.05,and the difference was statistically significant.The morphology of buds in the test group was better than that in the control group.5.The recovery time in the test group was significantly shorter than that in the control group(P<0.05).Conclusion:External application of Modified Sihuang ointment is safe.Modified Sihuang ointment can relieve postoperative pain,shorten wound healing time,reduce postoperative pain and improve the quality of wound healing. |
