| Objective:To explore the Problem – based Learning health education of integrated traditional Chinese and western medicine on the quality of life of patients with post-herpetic neuralgia,improve the health education ability of PHN patients,and provide reference for improving the pain management effect of PHN patients.Methods:A selection of 80 patients with PHN subacute phase who were hospitalized in the Pain Department of Wuhan Hospital of Integ rated Traditional Chinese and Western Medicine from July 2018 to June 2019.Using the principle of randomized control,they were divided into a control group and an observation group with 40 cases each.The control group implements routine pain care and path-based health education,and the observation group implements Problem – based Learning integrated traditional Chinese and Western medicine health education on the basis of routine pain care The protocol were such as:(1)the program was determined through discussing,modified after pre-test,then the unified standards were used in personnel training;(2)Following up the progress of disease treatment,collecting the main health problems of patients,no less than 20 minutes for each communication,and solving 1-2 problems with focus,paying attention to evaluate its effectiveness;(3)During Problem – based Learning integrative medicine health education implemented clinically,the main focuses included pain assessment,expression,intervention;Health maintenance of basic diseases;According to TCM syndromes,dialectical nursing was carried out in daily life,diet,emotional care,functional exercise and medication guidance.(4)Regular follow-up and rehabilitation exercises after discharge.All seleced patients were required.(1)Filling in SDS and SAS on the day of admission and one week after discharge.(2)Fill in PSQI on the day of admission and one month after discharge;(3)Fill in "Inpatient Satisfaction Survey" on the day of discharge.Descriptive analysis,chi-square test and independent T test were used for statistical analysis.Results:In the control group,2 cases were droped,and 38 cases were actually enrolled,including 21 males and 17 females,with an average age of(62.82±13.92)years old and an average course of disease of(62.00±18.01)days.In the observation group,1 case was droped,and 39 cases were actually enrolled,including 21 males and 18 females,with an average age of(64.15±13.92)years old and an average course of disease of(60.13±13.22)days.Among them,3 cases were of depression-heat type of liver meridian,9cases of spleen deficiency dampness type and 27 cases of qi stagnation and blood stasis type.There were no statistically significant differences in SDS,SAS and PSQI scores between the observation group and the control group on the day of admission(P > 0.05).After health education,there were statistically significant differences in SDS and SAS scores between the observation group and the control group one week after discharge(P < 0.01).One month after discharge,there was a statistically significant difference in PSQI scores between the two groups(P < 0.05).On the day of discharge,there was statistically significant difference between the two groups(P < 0.01).Conclusion:(1)the Problem – based Learning health education through integrated traditional Chinese and western medicine can reduce the depression and anxiety of PHN patients compared with the conventional path health education;(2)Problem-oriented integrative Chinese and Western medicine health education can improve the sleep quality of PHN patients compared with the conventional path health education;(3)Problem-oriented integrated Traditional Chinese and Western medicine health education can improve inpatient satisfaction of PHN patients compared with conventional path health education. |
