Objective: Observe the clinical efficacy of eye fumigation with traditional Chinese medicine(Sophora flavescens 、 Mentha haplocalyx)in the treatment of demodex blepharitis,evaluate its effectiveness and explore its mechanism of action.Methods: This study is divided into three parts.1.Experiment of killing demodex in vitro of Sophora flavescens and Mentha haplocalyx:There were two groups in this experiment,and in each group,there were60 fresh demodex.Joining the solution prepared by free-frying granules sophora flavescens and mentha haplocalyx=2:1 to implementation group 1.Joining the distilled water to implementation group 2.The number of domodex deaths was observed at 10、20、30、60、120minutes.2.Experiment of dosage proportion and dosage form selection of Sophora flavescens and Mentha haplocalyx:There were six groups in this experiment,and in each group,there were 60 fresh demodex.Among them,the implementation group 1,2and 3 was the free-frying granules group,and the dosage ratio of sophora flavescens and mentha haplocalyx was 1:1,1:2,2:1 respectively;the implementation group 4,5 and6 was the herbal medicine group,and the dosage ratio of sophora flavescens and mentha haplocalyx was 1:1,1:2,2:1 respectively.Joining the solution with different dosage proportion and dosage form of sophora flavescens and mentha haplocalyx,the number of domodex deaths was observed after 10 minutes.3.Clinical observation on the intervention effect of eye fumigation with traditional Chinese medicine(sophora flavescens,menthae haplocalycis)on demodex blepharitis:Strictly follow the inclusion and exclusion criteria,108 cases of demodex blepharitis diagnosed in the ophthalmology clinic of Affiliated Hospital of Chengdu University of traditional Chinese medicine from January 2019 to December 2020 were selected.According to the sequence of visit time,the patients were divided into three groups by random digital table method: test group 1[Eye fumigation with traditional Chinese Medicine(Sophora flavescens 、 Mentha haplocalyx)],test group2[Eye fumigation with traditional Chinese Medicine(Sophora flavescens 、 Mentha haplocalyx)combined with 0.5% metronidazole and sodium chloride injection scrubbing],control group[0.5% metronidazole and sodium chloride injection scrubbing].After 28 days of treatment,the symptoms,signs,tear break-up time,fluorescein staining of the cornea,sodium eyelashes demodex examination were observed.Results:1.Experiment of killing demodex in vitro of Sophora flavescens and Mentha haplocalyx:At each observation time,compared with implementation group 1 and implementation group 2,the difference was statisticant significant in the number of domodex deaths(P<0.05).The results showed that sophora flavescens and mentha haplocalyx had good killing effect on demodex.2.Experiment of dosage proportion and dosage form selection of Sophora flavescens and Mentha haplocalyx:(1)At 10 minutes,we observe the number of domodex deaths.Compared with implementation group 1 and implementation group2,implementation group 4 and implementation group 5,the difference was no statisticant significant(P>0.05);Compared with implementation group 1 and implementation group 3,implementation group 2 and implementation group3,implementation group 4 and implementation group 6,implementation group 5 and implementation group 6,the difference was statisticant significant(P<0.05).Therefore,different dosage ratio of sophora flavescens and mentha haplocalyx have influence on the killing effect of demodex in vitro,and the optimal dosage ratio is sophora flavescens:mentha haplocalyx=2:1.(2)Compared with implementation group 1 and implementation group 4,implementation group 2 and implementation group 5,implementation group 3 and implementation group 6,the difference was statisticant significant(P<0.05).Therefore,different dosage form have influence on the killing effect of demodex in vitro,and the free-frying granules is better.3.Clinical observation on the intervention effect of eye fumigation with5traditional Chinese medicine(sophora flavescens,menthae haplocalycis)on demodex blepharitis:(1)A total of 108 cases were included in this study,and finally 8 cases were dropped,included in 100 cases of which 35 cases in test group 1,34 cases in test group 2 and 31 cases in control group.(2)Before treatment,the differences had no statistical significance in the sexes,age,the Symptom score,signs score,the tear break-up time,fluorescein staining of the cornea,sodium eyelashes demodex examination,the difference was no statisticant significant(P>0.05),the three groups were comparable.(3)After treatment,the sexes,age,the symptom score,signs score,the tear break-up time,fluorescein staining of the cornea,sodium eyelashes demodex examination of the three groups were improved.Compared with before treatment,the difference was statisticant significant(P<0.05).(4)The curative effect: In the test group 1,the effective rate was 37.14%,the effective rate was 28.57%,and the total effective rate was 65.71%;in the test group 2,the effective rate was 55.88%,the effective rate was 35.29%,and the total effective rate was 91.18%;in the control group,the effective rate was 32.26%,the effective rate was38.71%,and the total effective rate was 70.97%.,the difference of the curative effect of the three groups after treatment was statisticant significant(P<0.05).The results showed that there was statisticant significant difference between the test group 1 and the test group 2 and the control group(P<0.05),and the difference between the experimental group 1 and the control group was no statisticant significant(P>0.05),The curative effect of test group 2 was better than that of test group 1 and control group,and the curative effect of test group 1 was equivalent to that of control group.Conclusion:(1)Sophora flavescens and mentha haplocalyx have good killing effect on demodex.(2)Different dosage ratio and dosage form of sophora flavescens and mentha haplocalyx have influence on the killing effect of demodex in vitro,and the optimal dosage form and dosage ratio is free-frying granules(sophora flavescens: mentha haplocalyx=2:1).(3)Eye fumigation with traditional Chinese Medicine(sophora flavescens、mentha haplocalyx),0.5% metronidazole and sodium chloride injection scrubbing have a certain effect on demodex blepharitis,but the combination of them is better,it can significantly improve the symptoms and signs of patients,prolong the tear break-up time,reduce fluorescein staining of the cornea,promote corneal epithelial repair,reduce the number of eyelash demodex.It has good clinical value,and it is worth further clinical application and in-depth research. |
