In recent years,the average life expectancy has increased significantly,and the proportion of the elderly population has also increased sharply.The elderly care problem caused by aging has become the focus of attention of the whole society.In order to better guarantee the quality of life for the elderly,different types of elderly care models provide a variety of elderly care options,among which the home care model has gradually become one of the main choices for the elderly.Because with the increase of age,the physical ability of the elderly declines.Under the dual effect of family environment and social environment,the ability of the elderly to take care of themselves in life has also become the main factor affecting their life satisfaction.This study uses the method of community work service,taking the M community in D city as an example,using questionnaire surveys and interviews to collect data,and using SPSS26.0 analyze the current situation,problems and needs of the semi-disabled living at home in the community,and using social support theory,embeddedness theory and social learning theory as direction,the community care model and community education method are mainly used to formulate service plans and carry out services.After about eight months of service,the goals of the service plan have been basically achieved and the effect is relatively significant.Most clients and their main caregivers are satisfied with the overall evaluation of the service and hope this service form can be continued.It is worth reflecting that there are also some shortcomings in the service,such as insufficient professional service personnel,shortage of service funds,and lack of support from corresponding social policies and regulations,which have caused some difficulties in service development and need to be continuously improved in the future development of community work,in order to provide better service and higher reference value.
