| Children are the future of a country or a nation.The healthy growth of children cannot be separated from the joint efforts of the government,society and family.Of course,good medical and health resources are also needed to guarantee it.However,at present,China’s pediatric medical resources and needs do not match,regional differences are too large,coupled with the urbanization process caused by the aggravation of population mobility,children to seek medical treatment in different places is prominent.For critically ill children who seek medical treatment in other place,they not only have to bear physical pain,but also face psychological and social discomfort.For their families,not only have to bear the huge economic pressure,illness and anxiety,but also face multiple difficulties such as the lack of social support in a strange place.Compared with the traditional medical model centered on the treatment of diseases in the past,the current concept of patient-centered "holistic" health is more advocated,that is,to promote patients’ physical,psychological and social adaptation to achieve an overall state of health balance.However,this goal can not be achieved by medical treatment alone.In recent years,the development of medical social work has the ability to promote patients’ comprehensive rehabilitation.Combined with professional theoretical models and integrated use of social work methods,it can largely alleviate patients’ psychological or behavioral problems caused by diseases,as well as difficulties in economic,family and care,social support system and so on.In this study,critically ill children from the inpatient department of the Department of Nephrology of Chongqing C Hospital were selected as the main research objects.Semi-structured interview and participatory observation were used as the main research methods.Case work method was used as the main intervention method.Supported by cognitive-behavior theory and crisis intervention theory,it explores effective ways to improve the plight of critically ill children seeking medical treatment in different places.In the introduction part,the paper mainly introduces the realistic background of the research and summarizes the research results of medical social work and patients who have to hospitalized away from habitual residence in domestic and foreign literature.The second part is a detailed elaboration of the research design,including the main research issues,ideas and theoretical basis.The third part is based on the observation of ordinary patients in the ward and the follow-up of the key patients,and analyses the general status quo of service targets and the real difficulties they face,from the four aspects of economy,medical treatment,psychological behavior and social support.The fourth part is the practical analysis of medical social work.Combined with the specific situations and personalized problems of the two children,we conducted professional intervention using crisis intervention mode and cognitive behavioral therapy mode in case work respectively,and demonstrated the service methods and effects according to the intervention process and interview records.The fifth part discusses the space and ability of medical social workers in the intervention of critically ill children in different places.It is believed that the plight of service objects is peculiar,and case work is the most effective way to intervene in their problems.Secondly,the reflection of this study is carried out from four aspects,including the inability to fully play the role and ability of medical social work interns,the limitation of services caused by the excessive focus on diseases in the hospital field,the limitation of resources,and the failure to make full use of the supervisory role.Finally,the study proposes that the professional services of medical social work should be based on the real needs of ill children,and establish a continuous and stable resource support system for service objects.Multiple system orientations should be appropriately used in the work process,and multiple intervention methods should be used comprehensively.In terms of professional development,it is necessary to reform traditional training methods and improve the overall quality of medical social workers. |
