Research On Competency Evaluation Of Family Doctor Embedded In Community-based And Home-based Elderly Care Services

As an inclusive mode of combining medical care with nursing care,family doctor embedding in Community-based and Home-based Elderly Care Services is mainly based on the mature family doctor contract system and provides integrated medical care service for the elderly with community family as the carrier.This service mode has low input cost and is conducive to improving the accessibility of Community-based and Home-based Elderly Care Services.To meet the needs of the elderly for medical and elderly care services.However,in the process of providing services at the present stage,family doctors generally have a single type of service and poor standardization,which makes it difficult to provide hierarchical and standardized medical maintenance services.Therefore,based on the heterogeneity of the needs of the elderly Communitybased and Home-based Elderly Care Services,this thesis constructs a competency model of family physician embedded in Community-based and Home-based Elderly Care Services.Based on the results and conclusions of the empirical evaluation,this thesis proposes countermeasures to enhance the competency of family physician embedded in Community-based and Home-based Elderly Care Services,so as to provide theoretical support for family doctors to provide more standardized and diversified services.This thesis takes the elderly and family doctors as the main research objects.First of all,questionnaire survey was used to explore the needs of the elderly for Communitybased and Home-based Elderly Care Services and the degree of demand,three different health types of elderly service needs were compared,the construction of a more comprehensive elderly Community-based and Home-based Elderly Care Services demand scale.Then,based on the community home-based care service demand scale,it is improved by literature reading and expert consultation,and converted into the specific competency index of family doctors.Then,the theoretical model of the competency of family doctors embedded in community home-based care service is constructed,and the competency questionnaire of family doctors embedded in community home-based care service is generated through the model index.Empirical test was carried out on the questionnaire issued by family doctors to determine the acceptability of the model and indicators.Finally,the index weight of the competency model of family doctor embedded in Community-based and Home-based Elderly Care Services was determined by AHP-PCA combination.The overall and various competency scores were calculated according to the weight value,and cross analysis was carried out to evaluate and analyze the competency of family doctors embedded in Community-based and Home-based Elderly Care Services.The empirical study found that the score of basic public health service ability of family doctors was the highest among the four competency dimensions,basically reaching excellent.The score of medical care service is slightly lower than that of basic public health service.The competency scores of family doctors in comprehensive services for the aged,personality and post role can only reach the passing level.Therefore,this thesis proposes to set up the selection mechanism of family doctor embedded in Community-based and Home-based Elderly Care Services.Establish a training system for family doctors embedded in Community-based and Home-based Elderly Care Services.Enriching incentives for family doctors.Construct performance evaluation and feedback mechanism of four aspects of strategy suggestions.
