The 2019 ICO China-related disease report pointed out that among adult women aged 25-59,cervical cancer incidence and mortality rate ranked second in malignant tumors.At the same time,modern medicine is undergoing a shift in the "bio-psycho-social" model and vigorously promoting the construction of healthy China in China.In this context,the self-management of cervical cancer patients has become the focus of attention of various academic circles.This study comprehensively considers the multiple dilemmas and needs of patients with cervical cancer.Under the guidance of acceptance and commitment therapy(ACT),through group work,we can work with patients to accomplish the three tasks of disease,emotion and role management,and improve the selfmanagement ability of patients with cervical cancer.First of all,this study analyzes the problems and needs of clients.In disease management,cervical cancer patients generally experience painful treatment experience,and patients often blame themselves or fatalism.In role management,patients will equate the patient’s role with "useless people" and independently substitute it,leading to the lack of sense of value and efficacy.On emotional issues,physiological conditions and role changes will bring involuntary emotional generation and changes to patients,and this kind of emotional fluctuation will run through the whole long-term treatment process.Therefore,the follow-up intervention of this study starts from the following needs of patients: long-term ability to identify and solve health problems,discuss and decide treatment plans;adapt to the patient’s role,continue normal life,establish new relationships;actively feel the moment,adjust emotions and the need to make changes in behavior.Secondly,this study selected 20 patients with stage I and II cervical cancer for intervention,and carried out process evaluation and outcome evaluation.There are 10 members in the intervention group and 10 members in the control group.In the intervention group,ACT was used for 6 times of group work intervention,while in the control group,routine medical social work services were provided.Before and after each intervention,the acceptance and action questionnaire(AAQ-II),cervical cancer patients’ quality of life assessment scale(FACT-Cx 4.0)and cancer patients’ selfmanagement assessment scale(CPSA)were used for pre-test and post-test,respectively.The evaluation shows that the empirical avoidance of cervical cancer patients can be significantly reduced,and the psychological flexibility,quality of life and self-management level can be effectively improved by accepting the group work intervention of ACT.Then,according to the evaluation results,the conclusion of this study is deduced.First,patients with cervical cancer are faced with pain experience,disorder of roles and emotional disorder.Second,patients with cervical cancer have the need to participate in decision-making,actively adapt to roles,adjust their emotions and make changes.Thirdly,ACT can effectively improve the psychological flexibility,quality of life and self-management level of cervical cancer patients.Fourthly,ACT provides new ideas for social work intervention in the field of cancer,and the effectiveness of the therapy is verified through interventions in specific groups.Finally,the author makes a further reflection on the research of social work intervention.First,the reflection in the process of intervention practice: the stigma of female cancer patients needs more research attention in the future;In the therapeutic group,social workers have experienced a series of trials,conflicts and adjustments;In out-of-hospital services,medical social workers’ services are lacking or even absent;ACT needs more practical and empirical evidence to support the effectiveness test of intervention for cervical cancer patients.Second,the reflection during the intervention evaluation: the collection of survey questionnaires is often delayed;After leaving the group,the patient only had a simple post-test without continuous follow-up.Third,the reflection on intervention ethics: following the ethical principles of social work,medical social workers often face many ethical conflicts,and need to further consider the ethical boundaries of group work in hospital services.Subsequently,the author further put forward the development suggestions of social work intervention in cervical cancer patients’ self-management mode from the perspectives of patients,doctors,social workers and society.
