Part oneThe study on the regularity of symptoms and signs of TCM in chronic renal failure. ObjectiveTo investigate prospectively the regularity of symptoms and signs of TCM in patients with CRF in multi-department of Nephrology in Guangdong province, and to provide evidences for further treatment. Method410 eligible CRF patients with age from 17 to 83 yeas old were enrolled in eight hospitals in Guangdong province, with the shortest cause 0. 2 month and the longest 28 years. It included 146 cases of chronic glomerulonephritis ,94 cases of diabetic nephropathy ,79 cases of benign arteriolar nephrosclerosis, 32 cases of obstructive nephropathy, 19 cases of polycystic kidney disease, 16 cases of renal atherosclerosis and 24 cases of other kidney diseases(the nephritis of Henock-Scholein purpura, lupus nephritis ect).The renal function of all cases:38 cases were in compensation stage, 104 were in decompensation,189 were in the stage of renal failure and 79 were in urimic stage. ResultDeficiency of spleen and kidney qi syndrome was common in root-deficiency syndromes in all stages of CRF, 341 cases were in this syndrome(83. 17%). Blood stasis and turbid pathogenic poison were vital factors in evil-excess syndromes, the blood stasis syndrome existed in the whole duration of CRF (onlythe degree was different). Along with the aggravation of the disease, turbid pathogenic poison syndrome came out gradually, it was a characteristic pathogenic factor in given phase of CRF, also an important cause which aggravated the disease. There were 194 cases with cold-dampness syndrome (47. 32%) and 143 cases with damp-heat syndrome(34.89%). ConclusionDeficiency of qi , blood stasis and turbid pathogenic poison syndrome exists in whole stage of CRF in different degrees , it is an important syndrome in patients with CRF in Guangdong province.Part twoThe study on clinical effect of the therapy of replenishing qi promoting blood circulation and eliminating poisonous pathogenic factors in CRF. Object iveTo investigate the clinical effect of the therapy of replenishing qi promoting blood circulation and eliminating poisonous pathogenic factors in CRF. Method180 enrolled patients with CRF were divided into two groups. The both were all taken quality low-protein and low-phosphorus diet and were given expectant treatment including depressing blood pressure, diuresis, redressing acidosis, anti-infection, redressing anemia and treating hyperparathyrodism ect. To make all patients' conditions stable above 2 weeks. The treat group was given Tongmai oral liquor, Niaoduqing granules, syndrome differentiation and treatment with TCM and herbal colon dialysis. The contrast group only was given Niaoduqing granules. The both were treated eight weeks and then symptoms and body signs, loboratory indexes were observed before and after the treatment. At last, the safety of this treatment were tested. ResultThe therapy of replenishing qi promoting blood circulation and eliminatinig poisonous pathogenic factors could improve fatigue, poor appetite, vomit and other clinical symptoms, there were obvious diffences compared with contrast group(P<0.01 or P<0. 05);it also could improve the laboratory indexs, for example Bun, Scr, Ccr, Hb, PT, APTT, FIB, CD3, CD4, CD8, IgG, Teh, LDL, ALB ect, there were conspicuous differences compared withcontrast group(P<0. 01 or P<0. 05). The total effective ratio of treat group was 86. 67%, it was obviously higher than the one of contrast group which was 56.67% (P<0. 05). ObjectiveTo investigate the therapy of replenishing qi promoting blood circulation and eliminating poisonous pathogenic factors' influence on serum fibrosis indexes in patients with CRF. MethodThe 50 enrolled patients were divided into two groups an random. The treatment liked the above. After 4 weeks treatment, the two groups' laboratory indexes PC—III, Col-IV, and LN (ELISA) were observed and then compared with the used. ResultThe therapy of replenishing qi promoting blood circulation and eliminating poisonous pathogenic factors could decrease serum PC—III, Col-IV, and LN(P<0. 01 or P<0.05), it existed conspicuous differences compared contrast group(P<0. 05). ConclusionThere are different degree fibrosis in kidney of patients with CRF, the therapy of replenishing qi promoting blood circulation and eliminating poisonous pathogenic factors could decrease the level of serum fibrosis indexes in patients with CRF.
