Study On Correlation Between TCM Syndrome Of Senile Depression With Constitution

Research objective:We will explore the relationship between senile depression and constitution through clinical research and recognize that individual differences have a extensive and an important practical applications on personalized prevention, treatment, rehabilitation and individuals.It will have an important clinical significance to prevent depression in the elderly or avoid the recurrence of depression and the mechanism and prevention of senile depression.Research methods:In accordance with the reference standard of syndrome differentiation, we research the pattern identification of 566 senile depression patients who accord with the receive standards. We judge the constitution type by TCM Constitution classification and judgment and find the most common constitution type;we summary out the law with Discriminant analysis and Factor Analysis;analysy the correlation between pattern identification and constitution,and find the most common pattern identification.Research result:(1)General information on patients with senile depression:By analysis patients with senile depression,we can summarized the main characteristics:①Lossing of spouse accounting for 49.12%,the reason may be the death of the spouse bring a large psychic trauma that can affect the appetite and sleep, so they can be easily induced depression;②Elementary and secondary education accounting for 62.9%, we considered the reasons.a, they have less interestand they lack of hope on life.b, Because of their poor education, they may have a low social status and a poor economy, these may result in lower self-evaluation,negative emotions.(2) Syndrome factor:There have 12 of 566 cases of senile depression pa (?)ts. The first three are deficiency of kidney, Yin deficiency, deficiency of spleen, and little difference between men and women. In addition to the se three,we can find that man patients have more syndromes, such as liver-fire, heart-fire, cardiac weakness syndromes caused by deficiency of the heart-energy;while we can see there have more syndromes in women, such as deficiency of vital energy, stagnation of liver-qi, phlegm, blood stagnation, (?)tomach-heat.(3) compound syndromes:566 patients with depression in in the elderly can be divided into the following 11 types of syndromes:Yin deficiency of liver and kidney, stagnation pf liver-qi and deficiency of Spleen, weakness of both heart and the spleen, the stasis of liver-qi, incoordination betwe-en liver and stomach, Yang deficiency of the spleen and kidney, stagnated vital energy clots the phlegm, stagnated vital energy clots the blood, exub-erant heart-fire and liver-fire, stagnation of liver-qi changes into (?)ver-fire,melancholy and depression impairing the Spirit. And the pattern of Yin deficiency of liver and kidney have the most patients. From the se-xual distinction, we can find the pattern of Yin deficiency of liver and k idney, the pattern of stagnation pf liver-qi and deficiency of Spleen, wea-kness of both heart and the spleen, the stasis of liver-qi accounting for a lot of propotion regardless of man or women. In addition to these, we ca n see man have a common pattern of exuberant heart-fire and liver-fire, stagnation of liver-qi changes into liver-fire, melancholy and depression impairing the Spirit. Women have a common pattern of incoordination betwee n liver and stomach, Yang deficiency of the spleen and kidney, stagnated vital energy clots the phlegm, stagnated vital energy clots the blood.(4) Characteristics of body constitution types in patients with depre-ssion in the elderly:through the analysis of body constitution types on (?) elderly patients with depression, we can found that most elderly patie (?)ts with depression do not simply have a type of constitution, they often carry two or more physical characteristics,not a single constitution. We can find constitution of deficiency of vital energy, constitution of Yang deficiency, constitution of Yin deficiency, constitution of stagnated vital energy,constitution of dampness-heat.(5)Relationship between constitution and syndromes:Multiple linear regression analysis showed that physical type have a significant impact. Specifically:①We can find that basic patterns of constitution of deficiency of vital energy are cardiac weakness syndromes caused by deficiency of the heart-energy and deficiency of vital energy;compound syndromes are Yin deficiency of liver and kidney and stagnation pf liver-qi and deficiency of Spleen.Because of Qi-deficiency and fail in Zang fu, so wo can see patients who have a constitution of deficiency of vital energy have a insufficienc y of the heart-energy. The heart governs mental activities. Referring to th e heart controlling the activities of spirit, consciousness, and thinking. So, if patients who have a constitution of deficiency of vital energy, they will show palpitation, insomnia, frequent dreams, amnesia, dizziness, pale com-plexion,inertia, ect.Qi,blood, Yin and Yang are inter-promoting and inter-impact. With the old.we can find flaming up of fire due to yin deficiency casused by asthenia of kidney. Yin deficiency of liver and kidney referring to a morbid condition of insufficient fluids of both liver and kidney;Patients who have a constitution of deficiency of vital energy often show abnormal in the activities of spirit, consciousness, and thinking, and the spleen will lost the function of transportation and spleen will fail to control blood.②We can find that basic patterns of constitution of Yang deficiency are deficiency of kidney and Yin deficiency;compound syndromes are stagnation pf liver-qi and deficiency of Spleen,Yang deficiency of the spleen and kidney.Because of Qi-deficiency and fail in Zang fu, so wo can see patients who have a constitution of Yang-deficiency have a deficiency of Yang, whic-h referring to a morbid condition caused by insufficiency and hypofuncti-on of yang. Manifested as intolerance of coldness.Yang deficiency include kidney-yang,spleen-yang and heart-yang. And kidney-yang have a most closely relation with Yang deficiency. Deficiency of kidney include not only deficiency of kidney-yang bt also insufficiency of kidney-yin. Deficiency of kidney-yang is a morbid condition caused by insufficiency of kideny-yang;Yin deficiency is refere to the disorder of yin-fluid deficiency. Because yin and yang depend on each other for coexistence, while being rooted in each other and making use of each other, so yang deficiency can caused yin deficiency.Constitution of Yang deficiency can cause yang deficiency of the spleen and kidney, and weakenness of the function of warmth, transformation and controlling;Due to the disorder of vital energy, there will show some problems in blood moving.③We can find that basic patterns of constitution of Yin deficiency are Stagnation of liver-qi and deficiency of kidney;compound syndromes are Yin deficiency of liver and kidney, incoordination between liver and stomach.Constitution of Yig deficiency is caused by the disorder of yin-fluid deficiency. And it can show internal heat due to yin deficiency.Yin deficiency can be found in heart, liver, spleen, lung, kidney, especially insufficiency of kidney-yin. Terminologyed inherent defect is the source of all deficiency, especially Yin deficiency. The stasis of liver-qi can be found if a people have a emotional depression and qi stagnation.⑤We can find that basic patterns of constitution of dampness-heat is deficiency of spleen;Compound syndromes are the stagnated vital energy clots the phlegm and incoordination between liver and stomach.Constitution of dampness-heat is caused by dampness-heat.Spleen likes dryness and dislikes dampness. If spleen-qi is vigorless failing to do transportation, water-dampness will occur,causing phlegm. Phlegm-dampness is apt to obstruct the functional activities of qi;It can affect the spleen's function in transportation and transformation.⑥We can find that basic patterns of constitution of accumulation of phlegm-dampness is phlegm-stagnated;Compound syndromes is stagnated vital energy clots the blood.Constitution of saccumulation of phlegm-dampness is caused by water stagnating at somewhere and becoming dense. Phlegm is a pathological product.Descending function of the lung can result accumulation of turbid phlegm,marked by oppressed feeling in the chest, profuse sputum.Due to phlegm-dampness can obstruct the functional activities of qi,so it will cause deficiency of vital energy. Dampness is yin pathogenic factor,characterized by heaviness, turbidity and viscosity, which is apt to obstruct the functional activities of qi. Dampness can affect the spleen's function in transportation and transformation. Like as constitution of accumulation of phlegm-dampness, phlegm and blood stagnation can interaction.⑧We can find that basic patterns of constitution of special intrinsic quality is stomach-heat;Compound syndromes is incoordination between liver and stomach.Constitution of special intrinsic quality is caused by a weak natural endowment and inheritance, including some diseases, physical defects, allerg-ic reactions. Due to the weak natural endowment, inheritanceand environmen-tal factors, drug factors, so there have many differences in physical char-acteristics, psychological characteristics, common manifestation, incidenc-e.Research Conclusion:The article shows that constitution, sexuality, syndrome factor both have effects to compound syndromes in elderly patients with depression. Especially constitution. Constitution of deficiency of vital energy, constitution of yin deficiency and constitution of yang deficiency are the important pathogeniccharacteristics. They have an important propotion in patients with senile depression. We can find that Yin deficiency ofliver and kidney, stagnation pf liver-qi and deficiency of Spleen are the common syndromes;Clinical investigation shows that patients with senile depression have some syndromes:deficiency of vital energy and yin deficiency, and they often express at the same time. With the old and feeble, deficiency of liver and kidney, feeble of Zang fu are the internal factor and the causing disease feature, the key is deficiency of kidney. This is different with the standpoint by some scholars which the stasis of liver-qi is the key. It may be have something to do with stage of age.
