BACKGROUND AND OBJECTIVEUlcerative colitis (UC) is a nonspecific inflammatory disease of rectum and colon, chronic recurrent type is most commonplace. Pathogenesis of the disease is not very clear. Environment, microorganism, heredity and immune factors may have relationship with it, among all of above, immune factors have been seriously concerned. Modern therapy strategy has been changed into inductive and maintenance care based on evidencebased medicine, but the side effects of western medicine restrict its clinical using, there are no positive appraisal for some brand-new treatment and medicine.Traditional Chinese medicine (TCM) has made development on the research of UC in the last 20 years, both on therapy and mechanism, Traditional Chinese Medicine also show its superiority in improving quality of life, lowering recurrence rate, but there are still many problems need to be resolved, such as using method of evidencebased medicine, under the guidance of Traditional Chinese Medicine theory, concluding characteristic of its Traditional Chinese Medicine syndromes, making profitable exploration and innovation. Combining with the development of modern medicine, making systematic and deep-going research, searching for its effect target, probing into its mechanism, we can adequately take advantage of Traditional Chinese Medicine.My tutor Tian delu consider UC active phase of chronic recurrent type has specific characteristic syndrom type of TCM, that is: deficiency of spleen yang, retention of damp-heat and blood are common, so its clinical characteristic is simultaneous occurrence of cold and heat syndromes, simultaneous occurrence of deficiency and surplus syndromes, we could get good curative effects using warm purgation. Thus, I selected UC active phase of chronic recurrent type as the research objective, considering deficiency of spleen yang and retention of damp-heat and blood, using the treatment of differentiating syndromes combined with differentiating disease, combining microcosmic differentiating syndromes. Observing the change of clinical symptom and mucous membrane lesion after using warm purgation, observing evolution of syndromes and relationship between the grade of disease and unbalance of cell-mediated immunity, observing the change of cytokine before and after treatment, inquiring its pathology mechanism centered around immune impairment. METHODSThe study was strictly designed under the guidance of GUIDELINE FOR NEW DRUGS CLINICAL TRIALS (1997 Edition 3) Guidelines for Treament of Chronic Unspecific Ulcerative Colitis with Chinese Materia Medica. In combination with actual clinical situation, adopting random positive control method, on the basis of diagnosis criterion established on Inflammatory Bowel Disease Seminar at chengdu in 2000, 43 case of chronic recurrent type in UC active phase was selected from out-patient clinic(OPD) andward of Dongzhimen Hospital, Dongfang Hospital, Guanganmen Hospital, treated group received decoction for warming spleen (rhubarb prepared aconite root 12g, dried ginger 10g, tangshen 10g, liquorice root 6g), control group received sulfasalazine (SASP), (4-6g/d). Both treatment course was 4 weeks. main observed index: 1. Therapeutic effect, including clinical general effect, change of TCM syndrome, change of main clinical symptom (diarrhea, hematochezia, abdominal pain), therapeutic effect of mucous membrane under endoscope, therapeutic effect of mucous membrane under microscope, the change of clinical activity index, CAI, endoscopic index. 2. Immue index of both groups before and after treatment, CD4+/CD8+/CD3+of peripheral blood, serum nitric oxide(NO), interleukin-8 (IL-8), tumor necrosis factor alpha (TNF-(), epidermal growth factor (EGF), NO,TNF-(,IL-8 and EGF were also detected in 20 normal persons as normal control.RESULTSObservation1.
