Objective: In order to explore the action, mechanism and clinical significance of inflammatory medium in the episode of cerebral infarction and its serum change in course and different volume infarction of disease.Methods: 50 patients with cerebral infarction were selected as the experimental group including 27 male cases and 23 female cases . The 50 cases were divided into three groups according to the volume of infarction, 18 cases were small infarction group ( the first group ), 16 cases were middle infarction group ( the second group), 16 cases were large infarction group (the third group). We also selected 30 normal persons as control group , including 17 males and 13 females . The level of nitric oxide (NO), Tumor necrosis factor- a (TNF- a ), Interleukine-1 P (IL-1 P ), Interleukine-6 (IL-6), Intercellular adhesion molecule-1 (ICAM-1), Interleukine-8 (IL-8), Transforming growth factor- P i (TGF-P i) in serum and some clinical indexes were measured at the same time by ELISA method.Results: The level of NO in serum in cerebral infarction group at different course group compared with that of controlgroupo The serum NO level of the first group was low remakably at acute stage(P<0.05)and was raised remarkably at 7th day (P<0.05), and it was no significant difference at 21st day (P>0.05). The concentration of NO in cerebral infarction group at 7th day was remarkably higher than other times (P<0.05).The concentration of TNF- a and IL-1 P in serum compared with those of control group at different course of ACI were found to be increased dramatically at acute phase (P<0.05). However the level of ICAM-1 and IL-6 were raised remarkably at acute phase (1st day , 3rd day ,7th day) ,and were no significant difference at recovery phase (21st day) (P>0.05). The level of TNF- a and IL-1 0 and ICAM-1 and IL-6 in serum at other times of ACI were respectively higher than that of 21st day group(P<0.05).The levels of IL-8 at the period of ACI were higher than that of control group remarkably (P<0.05). At the period of ACI the serum levels of IL-8 in respective cerebral infarction group was no significant difference (P>0.05).The serum levels of TGF- P ] at different course group of ACI compared with those of control group were reduced strikingly at acute phase (P<0.01) and were no significant difference at recovery phase (P>0.05). At the period of ACI the concentration of TGF- {3 | were remarkably lower than that of 21 st day group.The levels of NO and TNF- a and IL-1 3 andICAM-1 and IL-6 and TGF- P , at the different course of ACI in the different volume group were raised strikingly with the increased size of infarction volume . The concentration of TGF- P i and IL-1 P and IL-6 at the first group compared with those of the third group were significant difference (P< 0.05 and they were no significant difference contrasted with those of second group . The content of NO at the first group compared with that of the third or the second group was strikingly difference(P < 0.05)and was no significant difference at the second group contrasted with that of the third group(P>0.05). The concentration of TNF-a and ICAM-1 in serum of ACI between the third group and the first ,the second group were significant difference(P<0.05). The levels of NO and TNF- a and IL-1 P and ICAM-1 and IL-6 and TGF- P ] at the distinct course and volume group were notable difference with that of the control group(P< 0.05).Conclusion: There was disturbance of neuro-inflammatory/irnrnune-endocrine function in ACI patients . This result indicated that NO and TNF- a and IL-1 P and ICAM-1 and IL-6 and IL-8 and TGF- P , in serum played a role in the acute cerebral infarction's occurrence and development of inflammatory and immune process , as well as they are ones of pathologic basis of ACI as immunity agents.With the dynamic observation of serum NO in ACI , itdisplayed that the content of NO in 1st day, 3rd day increased gradually from the concentration below normal level, and at peak in 7th day , then descent...
