Object: To evaluate the clinical effect of HuoXueXiaoBan Chongji used as inhibitor to prevent the formation of epidural scar after discectomy.Methods: All subjects stemmed from the inpatients in department of orthopaedics from Apr, 2001 to Dec, 2002 who sufferde from PLID. 45 patients were divided into three groups randomly: HuoXueXiaoBan Chongji group(Herb group), diclofenac sodium group (Western Medicine group) and vacuity control group (Control group) and underwent classic discectomies. 4. Og cefoperazone sodium was given to all postoperative patients each day. The first day after operation HuoXueXiaoBan Chongji (It included 8 herbs: dan-shen root, Szechwan lovge rhizome, peach seed, safflower, yuan-hu, earth-worm, Chinese angelica, membranous milkvetch root) was given to Herb group' s patients TID and a packet one time which had 15g herbs till the 2nd week. 75mg diclofenac sodium (Votalin, which was made in Beijing NuoHua company limited ) was given to each patient of Western Medicine group. Pol lowing markers were observed preoperatively and in the 1st week and the 2nd week postoperatively: 1. Hemorheology. 2. The content of inflammation medium: PGEL.. 3. The content of urinary hydroxyproline(HOP). Enhanced CT scan was examined in the 8th week. The clinical effect of operation was judged according to the norm of Lu Yupu.Results: 1. Hemorheology: All markers of patients in Control group rised in the 1st week postoperatively and the condition continued till the 2nd week. Comparedwith Herb group and Western group there was very significant difference in two weeks (P<0. 01). However there was no difference between Herb group a'nd Western Medicine group(P>0. 05). 2. The content of PGE2: Compared with the preoperative content, it rised notably in control group (P<0. 01). On the contrast it reduced in both Herb group and Western Medicine group (P<0.01). There was very notable difference between Control group and Herb group, Control group and Western Medicine group(P<0. 01). No difference existed between Herb group and Western Medicine group (P>0. 05). 3. The content of urinary HOP: There was no difference in three groups in the 1st week(P>0.05). The content rised in the 2nd week compared with it preoperatively and in the 1st week (P<0. 01). There was significant difference between Control group and Herb group, Control group and Western Medicine group (P<0. 01). 4. Enhanced CT scan : Protrusion with different scope and and density around dura mater were observed in all groups but enhanced extent was different (P<0. 05) in the 8th week. 5. To judge the clinical effect of operation after 8 weeks: The excellent rate was different in three groups (P<0.05).Conclusion: 1. HuoXueXicioBan Chongji could reduce the epidural scar formation after discectomy. 2. HuoXueXiaoBan Chongji could be used to prevent the epidural fibrosis and adhesion after discectomy. Compared with diclofenac sodium it had no notable side effects and was safer in clinical application. 3. HuoXueXiaoBan Chongji could be popularized to other spine surgeries to reduce the incidence of FBSS and produce great social effects and economic returns.
