Objective:To explore changes of the levels of interleukin-1(IL-1),intercellular adhension molecule-1(ICAM-1),C creactive protein(CRP) in blood serum and mumeration of leukocyt of acute cerebral infarction patients,and to explore the relation of inflammatory reaction and brain injury after ischemia.Methods:A ccording to bulk of infarction,acute cerebral infarction patients are divided into three groups:big bulk group,middle bulk group and small bulk group.Their levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt are detected at the time of going to hospital(within 72 hours),at the end of 1 week and 4 weeks after episode. At the same time,collect the crowd in danger of stroke as the contrast group.Result:(1)At the earlier period of cerebral infarction,the levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt have positive correlation with the bulk of infarction ,approaching to big bulk group are significantly higher than middle bulk group(P<0.05), middle bulk group are significantly higher than small bulk group(P<0.05);(2)At the end of 4 weeks, the levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt of big and middle bulk group descent significantly, and in the stage of recovery they keep descenting significantly except IL-1. (P<0.01) While the marks of small bulk group have no significant changes though the whole course except CRP(P<0.05);(3)Comparing with the contrast group, the four inflammation markers of cerebral infarction group are significantly higher only at acute period(within 72 hours ,at the end of 1 week).In the stage of recovery they have no significant difference with the contrast group except CRP(P<0.05); Conclusions:(1)At the acute period of cerebral infarction,the levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt have positive correlation with the bulk of infarction .They reflect the degree of brain injury after ischemia,but their specificity need to be investigated more deeply.(2) At the acute period, the levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt of big and middle bulk group step up significantly,and in the stage of recovery they descent,which reflect that inflammatory reaction plays an important role in acute brain injury in larger size infarction group.(3) Comparing with the contrast group, the levels of IL-1,ICAM-1,CRP in serum and mumeration of leukocyt of cerebral infarction group are significantly higher only at acute period, while in the stage of recovery they have no significant difference except CRP,which reflect that the levels of IL-1,ICAM-1 in serum and mumeration of leukocyt probarbly are only significant at acute period ,while the bottom level of CRP may reflect lasting flammatory reaction after stroke. (4) Inflammatory reaction has a close relation with acute brain injury after ischemia and reflect the development of cerebral infarction.By detecting inflammation markers ,we can judge the degree of acute brain injury after ischemia,and take measures to save "semi-dark band" brain constitution in order to improve prognosi.
