objective : To observe the effects of the granula of penetrating bone and removing pain on prevention and treatment of osteoarthritis and investigate the effects of mechanism from some aspect, such as: cartilage cytokine, oxygen free radical, blood rheology and microcirculation. Method: The healthy rabbits of new Zealand aged 6 six months, half male and half female, were randomly divided into two large groups, i.e: the normal group and the operation group. According to the improved operational method of Hulth, we had operated the animals which belong to the operation group. Then they were randomly divided into five small groups, i.e. the ZhuangGuGuanJieWan group (the control group), low-dose group( I experimental group), midst-dose group (II experimental group), high-dose group(III experimental group) and the model group. The animals which were operated were forced to locomote an hour after they were fed adaptatively a week ago. In order to achieve the effect of prevention and treatment of osteoarthritis during the course, all animals were enforced proportional intragastric administration with the water-solution of the granula of penetrating bone and removing pain, the water-solution of the ZhuangGuGuanJieWan and the isotonic Na chloride. 2 months later, we need collect the article synovial fluid and blood, and detect such indicatrix as IL-l,MMP-3 and NO of all animal in every group. 4 months later, the two rabbit in every group were randomly chosen to photographs in the orthophoria and lateral position. Before all animals were sacrificed, such specimen as synovial membrane , the article synovial fluid, blood andarticle cartilage was collected, and to be detected IL-1, MMP-3, NO and SOD, and to be observed article cartilage under the electron microscope.Result: (l)The report of CR corresponds to the sequentiae condition. In the model group, there is obviously narrowing between the arthral wall blank. The articular facet is obviously rough and deformation. There is the obviously osteophyte at the verge of the articulus, and bone density is obviously dense under the cartilage sending down fishbone. About the retrograde variation of cartilage, the model group is more severe than all the experimental group .The III experimental group is the lighter than the other group in the arthral retrograde degree. (2) The general morphous observation. In the respect of the cartilage, in the normal group ,the joint cartilage is light blue and white and semitransparent. It's surface is smooth, and gloss is bright, there is no crackle and softening focus. The joint edge is regular and neat; In the model group and the I experimental group, its surface is rough, its edge is irregular, there is some obvious osteophyte on the cartilage. The cartilage flawed was damaged deeply and reaches the cartilage middle layer, even deep layer. The cartilage sclerotin is peeling and exposing osseously under the cartilage; In the control group and the II experimental group, the cartilage color obviously lost inhere gloss. The color and luster become dark, becoming deep yellow, what the cartilage is touched is relatively soft. In the III experimental group, the joint cartilage surface color turns into simple and white. The surface ciliary has coarse changes. The joint edge is regular and relatively neat, there is no osteophyte. The cartilage makes the osteosclerosis (Increasing thick). In the respect of the membrana synovialis and synovial fluid. In the normal group, membrana synovialis surface is smooth and glazing, synovial fluid is nankin, brilliant glutinosity fluid, liquid quantity is less, viscidity is sky-high. In the model group, the articular capsule and synovium is hyperplasia, plump, conglutination, the synovial tufts of membrana synovialis increases thickly. The III experimental group capsula articularis and synovium exist light-degree hyperplasia, conglutination, synovial fluid is less than the I experimental group. The result of the control group, the II experimental group is between the I experimental group and the III experimental group. (3) Greatsignificant contrast(p<0.01) were found among the results of I experimental group, II experimental group, III experimental group, the control group , the model group and those ofthe normal group, in respect of the contents about IL-l,MMP-3,N0 and SOD (p<0.01); The significant contrast(p<0.05) were found among the II experimental group, the control group and the model group, in respect of the contents about IL-1, MMP-3, NO and SOD; There is great significant contrast(p<0.05)between III experimental group and the model group at 8 weeks, great significant contrast (p<0.01) at 16 weeks,in respect of the contents about IL-1,MMP-3,NO and SOD; There is significant contrast (p<0.05)among the I experimental group , the control group and III experimental group about IL-1,MMP-3,NO and SOD. (4) The transmission electron microscope show: the surface layer collagen fibers is coarse in the experimental group , the model group is disorder than the experimental group on each layer collagen fibers. The chondrocyte is atrophy. Contrast the normal group, the surface projection is increase. Intranuclear different dye-substance is increasing, irregular. RER (rough endoplasmic reticulum) expand; it is thus clear that lipid droplet and microfilament in the intracytoplasm. In the all, the experimental group are superior to the control group. Conclusion:1 In this experiment, after the rabbits were operated with the way of the Hulth, the model of the OA were established succeedly. According to the Computed Radiography(CR) plain film, the general observation of the articular cartilage, the observation of electron microscope ,some biochemical indicator.such as: IL-K MMP-3, NO and SOD , it was demonstrated succeedly.2 The granula of penetrating bone and removing pain was triturated by such the theory as liver controlling tendon, the kidney being in charge of bone, spleen controlling extremities and muscle. Its guidance thinking is that the drug can reinforce liver, tonifying kidney, invigorate the spleen, expelling wind ,removing cold, dehumidify,tendon relaxtion,activeation of blood circulation and remove meridian obstruction. Then in order to achieve the curative purpose, so as to decrease the inflammatory reaction, improve part blood circulation and postpone degenerative cartilage.3 The granula of penetrating bone and removing pain can control the level of IL-K MMP-3 > NO and SOD. The drug regulate and affect the pathogenesy of OA from the various aspect, such as cartilage cytokine, oxygen free radical, blood rheology and matrix metalloproteinases.
