Objective: To explore the expression of CD15, CD44v6, nm23 and and its clinicalsignificance of thyroid neoplasms, and the relationship between the expression of CD15,CD44v6, nm23, microlymphatic vessel density with lymphatic metastasis of human thyroidcancer.Method: The expression of CD44v6, CD15, nm23 antigen in 71 thyroid cancers, 31adenomas, and 10 normal thyroid tissues were investigated by immunohistochemical pv-9000and tissue microarray technique, and micro-lymphatic density were counted. The results weretested by statistics methods which were chi-square test, t-test, Spearman' rank correlationusing SPSS11.5.Results:①The hyper-expression rate of CD44v6 in thyroid cancer was 53.5%, whichwas significantly higher than 29.0%, 20.0% in adenomas and normal thyroid tissues(X~2=7.775, P<0.05). No correlation was observed between expression of CD44v6 andhistological type of thyroid cancer. The distinction between groups with different tumor sizewas significant (X~2= 5.960,P<0.01). The hyper-expressions of CD44v6 in cases withlymphatic metastasis, and of stageⅢ~Ⅳwere higher than that in those without lymphaticmetastasis and of stageⅠ~Ⅱ(X~2=7.398, 4.070, P<0.05). The hyper-expressions of CD44v6in papillary thyroid carcinomas were higher than that in undifferentiated carcinomas and theother three types (X~2=4.551, 4.125, P<0.05).②The ratio of expression of CD15 in thyroidcancer was 46.5% while that of adenomas and normal thyroid tissues was 22.6%, 20.0%. Thedistinction between the three groups was marked(X~2=6.693, P<0.05); The hyper-expressionrate of CD15 in papillary thyroid carcinomas were higher in contrast to the other three types(X~2=5.073, P<0.05). The difference between lymphatic metastasis group and group withoutlymphatic metastasis was significant (X~2=7.138, P<0.01), as well as between the group ofstageⅠ~Ⅱand stageⅢ~Ⅳ(X~2=5.894, P<0.05).③The significant difference of thehyper-expression of nm23 existed between the thyroid cancer and adenomas and normalthyroid tissues (X~2=11.224, P<0.01). And the hyper-expression of nm23 between lymphaticmetastasis group and group without lymphatic metastasis was different (X~2=5.328, P<0.05), also between the group of stageⅠ~Ⅱand stageⅢ~Ⅳ(X~2=4.669, P<0.05). Thehyper-expression rate of nm23 in papillary thyroid carcinomas were higher than the otherthree types (X~2=3.896, P<0.05), while there is no statistical significance in groups withdifferent tumor size.④MLVD of node positive group was much higher than that of nodenegative group (t=5.619, P<0.01), and group of stageⅢ~Ⅳwas higher than groups of stageⅠ~Ⅱ(t=5.053, P<0.01). MLVD of papillary thyroid carcinomas and follicular carcinomaswere higher in contrast to undifferentiated carcinomas(t=3.581, 2.272, P<0.01).⑤With theincreasing expression of CD44v6, the expression of nm23 decreased. There was negativecorrelation between them(r=—0.539, P<0.01).Conclusion:①The hyper-expression of CD44v6,CD15,nm23 is related with thyroidcancer development, some pathological characters and biological behaviors closely.②Thehyper-expressions of CD44v6 in thyroid cancer can be an index to the degree of malignancyand worse prognosis, we can use it to estimate prognosis and direct therapy.③Theglucoprotein CD15 is changed in quality and amount with the development of thyroid cancer,so the expression of CD15 can be regarded as a parameter for evaluating the degree ofmalignancy, biological behavior and prognosis of thyroid cancer.④nm23 plays a role ininhibiting proliferation of tumor cells, inducing tumor cells differentiation, negativelyregulating cells growing and metastasis.⑤There was negative correlation between CD44v6and nm23 in thyroid cancer, it shows that the imbalance expression of CD44v6 and nm23 isone of the factors affecting the prognosis.⑥The higher MLVD,hyper-expression ofCD44v6,CD15 are positively correlated with lymph node metastasis, and nm23 is negativelyassociated with lymphatic metastasis. It shows that there is synergistic effect of multiple geneand multiple factor in the process of metastasis.⑦Tissue microarray is an efficient, lowconsume and strong comparability technique in study of human pathology.
