The Concentration Changes And The Clinical Significance Of Serum Insulin-like Growth Factor-1, Procollagen Type Ⅲ In Hypertensive Patients With Left Ventricular Hypertrophy

Objective:To investigate the relationship between insulin-like growth factor-Ⅰ(IGF-Ⅰ)or procollagen typeⅢ(PCⅢ)and left ventricular hypertrophy(LVH)And the relationship between insulin-like growth factor-Ⅰ(IGF-Ⅰ)and procollagen typeⅢ(PCⅢ)in patients with essential hypertension(EHP).Methods:In 28 normal individuals and 61patients with EH,the serum level of IGF-Ⅰor procollagen typeⅢ(PCⅢ)was measured using radiolinked immunoassay,and left ventricular mass index(LVMI)was calculated based on parameters determined by echocardiography.Differences in LVMI and serum IGF-Ⅰor procollagen typeⅢ(PCⅢ)among groups of normal controls,patients with EH alone or LVH- complicated EHP(EHP-LVH)were analyzed with univariate ANOVA.Pearson's linear correlation was employed to identify the possible relationship between serum IGF-Ⅰand LVMI.Results:The IGF-Ⅰlevel was significantly higher in EHP patients than in normal controls(P＜0.01),as was in EHP-LVH patients than in EH patients(P＜0.01)or than in normal controls(P＜0.01).The level of IGF-Ⅰshowed positive correlation with LVMI(r=0.640,P＜0.01).The PCⅢlevel was significantly higher in EH patients than in normal controls (P＜0.01),as was in EHP-LVH patients than in EHPpatients(P＜0.01)or than in normal controls(P＜0.01).The level of PCⅢshowed positive correlation with LVMI(r=0.423,P＜0.01.),and the serum IGF-Ⅰwas not correlated with PCⅢin EHP group and EHP-LVH group(P＞0.05).Conclusion:1.The positive correlation between serum's IGF-Ⅰor PCⅢand EH or EHP-LVH suggests that IGF-Ⅰor PCⅢmay be involved in the pathophysiological process of essential hypertension,and probably play a very important role in the development of myocardial remodeling. 2.IGF-Ⅰshowed no correlation in the development of hypertension and myocardial remodeling with PCⅢin EHP group and EHP-LVH group that suggests that both humoral factor IGF-Ⅰand PCⅢmay be play its role in the cardiovascular system as the two independent role in essential hypertension and left ventricular hypertrophy patients.
