Study On The Changes Between Tongue Cell Apoptosis And Microecology And Influence Of Chinese Herbal Compound Medicine Of Clearing Damp-Heat On Damp-Heat Syndrome In Spleen And Stomach

Objective: To analyze the microcosmic mechanism of effects of Chinese herbal compound of clearing damp-heat on the coating of spleen-stomach damp-heat syndrome by observing changes of index in tongue epithelial cell apoptosis and micro-ecology,impact of Chinese herbal compound of clearing damp-heat interventions.Methods: Fifty cases of spleen-stomach damp-heat syndrome with yellow- greasy coating as the experimental group,thirty cases of health subjects with light red tongue and thin white fur as the normal control group. The experimental group was randomized into treatment group(n=30)treated by Chinese herbal compound of clearing damp-heat(modified Huopuxialing decotion,MHD)and control group(n=20)treated by domperidone. The two groups were treated for two weeks(fourteen days)and required to return visit once a week. The clinical effect was evaluated with the changes in coating and syndromes,and then scrape tongue coating to test apoptosis index,total bacterial count,bacterial concentration,flora diversity of tongue epithelial cell as well.Results:1.The levels of total bacterial count,bacterial concentration of Gram- positive long-chain coccus,Gram-positive small coccus,Micrococcus and Gram- negative small bacillus,types of coccus and epiphyte before treatment in experimental group were significantly higher than those in the normal control group,but the apoptosis index of tongue epithelial cell was markedly lower than that of the normal control group and there was significantly differenc(eP<0.05).2.The total bacterial count of yellow greasy fur and the severity of tongue classification in experimental group were positively correlated,while the apoptosis index of tongue epithelial cell and severity of tongue classification were negatively correlated.3.There were significantly differences in integration of symptoms and tongue coating of patients with spleen-stomach damp-heat syndrome before and after treatment by Chinese herbal compound of clearing damp-heat in treatment group(P<0.05). There was significant difference compared with the western medicine control group(P<0.01). Five cases were clinical cure,twelve cases were remission effective,ten cases were effective and three cases were ineffective. The total effective rate was 90%. No obvious adverse reaction was seen. 4. The total bacterial count of tongue fur,bacterial concentration and diversity were decreased(P<0.01,P<0.05)and tongue epithelial cell apoptosis was increased(P<0.01)and epiphyte restored to a single composition after treatment by MHD in treatment group. There was significant difference compared with the western medicine control group(P<0.01,P<0.05), while no significant difference compared with the normal control group(P>0.05).Conclusion:1.Significant reduction of apoptosis of tongue epithelial cell and serious imbalance in micro-ecology exist in the spleen-stomach damp-heat syndrome. The yellow greasy coating was heavier, the reduction of apoptosis of tongue epithelial cell would be more significant and the imbalance in micro- ecology would be more serious.2.Chinese herbal compound of clearing damp-heat can not only get good clinical effects on spleen-stomach damp-heat syndrome but also improve the yellow greasy coating obviously . Chinese herbal compound of clearing damp-heat can maintain the balance state among tongue epithelial cell growth,differentiation and apoptosis by promoting tongue epithelial cell apoptosis increased, and then reconstruct the normal tongue flora by prompted the total bacterial count of tongue fur,bacterial concentration and diversity decreased and epiphyte restored to a single composition. Finally recover the normality of tongue.
