Objective:To evaluate the the protective effects and mechanism of yishen capsule in the kidney of in Streptozotocin-induced rats with diabetic nephropathy via decreasing the express of NF-κB and provide experimental theory basis for the yishen capsule clinical application of diabetic nephropathy.Methods:40 normal Wistar rats were randomly divided into four groups:Normal control group (control group), DN model group (model group), yishen Capsule treatment group, Losartan treatment group. After diabetic model establishment, the rat in group of Yishen Capsule were gavage given 625 mg-kg-1·d-1, Losartan treatment group were gavage given 20 mg-kg-1·d-1, and for the rats in other groups the same amount of fresh water was used. Then we detected the blood sugar of each group rats in 2w,4w,6w,8w and 12w. All the rats received daily gavage for 12 weeks. After 12 weeks,The urinary protein in 24h and the level of blood sugar, serum creatinine and urea nitrogen were measured. The renal pathology was detected under light microscope. The express of MCP-1 and NF-κB were detected by immunofluorescence. Measurement data to mean±standard deviation (x±s), said multiple comparisons using single-factor analysis of variance, using SPSS12.0 statistical package for data analysis, P<0.05 are statistically significant, P< 0.01 indicated that the difference was statistically significant.Results:1. The level of random blood glucose was all higher than 16.7mmol/L in model group, yishen Capsule treatment group and Losartan treatment group on 3 days after STZ Intraperitoneally. The blood sugar of model group was higher than that of control group in 2w,4w,6w,8w and 12w(P<0.05), as these in yishen Capsule treatment group and Losartan treatment group. The control group has a stable blood sugar level. In 2 w, rats of model group lost body weight, followed by trand of reduced body weight in the range, so as the rat of yishen Capsule treatment group and Losartan treatment group; the control group of rats gained weight during the experiment. In 12w, Scr, BUN and urinary protein quantity in 24h of model groups was higher than that of control group (respectively, P<0.05). While yishen Capsule treatment group and Losartan treatment group have a significance lower level (respectively P<0.05), but there was not significance different between the two group.2. Morphological changes of renal tissue the renal of rats in control group has no significance changes by light microscope, and the distribution of glomerular extracellular matrix and Mesangial cells cell was normal. Moreover there were normal capillary cavity, normal tubules and small tubes-interstitial zone, tubular epithelial cells were square. Glomerular sclerosis and fibrosis were not observed. We can see that basement membrane thicken, widened mesangium, increased mesangial matrix in model group by light microscope. Benazepril and yishen Capsule can improve ameliorate the renal pathomorphology change.3. The immunofluorescence study of MCP-1 and NF-κB showed that the expressions of them were a small quantity in renal cells in control group, In model group the expression of NF-κB was higher in renal tubular epithelial cells and renal glomerulus cells than control group, the expression of MCP-1 was obviously increased in renal glomerulus cells of model group. Generally the expression of MCP-1 and NF-κB was higher in model group than that in control group (respectively P<0.05). While yishen Capsule treatment group and Losartan treatment group have a moderately expression in both intracytoplasm.Conclusion:1. yishen capsule can significantly reduce the injury of renal function and the pathological extent of renal glomerulus injury in DN rats.2. yishen capsule can protect DN rats via decreasing the express of MCP-1 and NF-κB.
