ObjectiveThis study was designed to approach the expression of Ezrin and CD44V6 inmetasta tic bone tumor and the correlation between the two with immunohistochemistry. Inthe study, RT-PCR method was used to test the expression level of Ezrin mRNA of huma nspecimen of metasta tic bone tumor from gene level.Clinica l pathologica l data was ana lyzedassocia ted with the expression level of Ezrin protein and gene for the correlation. Theresearchers also probed into the clinica l significa nce of Ezrin upon incidence anddevelopment of metasta tic bone tumor.Method40 cases of bone tumor specimens from operations during March 2008 and March2009 were collected from Xinq iao Hospita l of Third Milita ry Medica l University. 12 casesof specimens were from benign lesion of bone while the other 28 cases from metasta ticbone tumors. The protein expression of Ezrin and CD44v6 was tested withimmunohistochemistry. RT-PCR method was used to test the expression level of EzrinmRNA. Software SPSS 13.0 was used in statistica l treatment.Staining positive and strongpositive were included in positive group, compared with the negative group. Enumerationdatas were ind ica ted with rate s. The x2 test was used to test the relationship betweenimmunohistochemistry,RT-PCR results and clinica l pathologica l parameters. Spearma ncorrelation ana lysis method was used in correlation ana lysis . The difference wasstatistica lly significa nt when P<0.05.Results1. Positive expression rate of Ezrin in metasta tic bone tumors was 85.7% (24/28) .Positive expression rate of Ezrin in benign lesions of bone was 16.7% (2/12). Thedifference was statistica lly significa nt (x2=17.603, P=0.000). 2. Positive expression rate of CD44V6 in metasta tic bone tumors was 78.6% (22/28) .Positive expression rate of CD44V6 in benign lesions of bone was 25.0% (3/12) . Thedifference was statistica lly significa nt (x2=10.286, P=0.001).3. In all specimens of metasta tic bone tumor, positive expression rate of Ezrin proteinwas 85.7% (24/28), while positive expression rate of CD44V6 protein was 78.6% (22/28).21 cases ind ica ted positive both on Ezrin and CD44V6 and 3 cases ind ica ted negative bothon Ezrin and CD44V6. So there was a positive correla tion between Ezrin protein andCD44V6 protein (r = 0.533, P <0.05).4. In all specimens of metasta tic bone tumor, 22 cases ind ica ted positive expression ofEzrin mRNA, with a positive rate of 78.6%. In the benign lesion of bone control group, 2cases ind ica ted positive , with a positive rate of 16.7%. The difference was statistica llysignifica nt (x2=13.413, P=0.000).5. Group comparison on difference of Ezrin protein and mRNA expression wasperformed accord ing to sex, age, sites of metasta tic tumor, whether prima ry ma ligna ncyexists, and the differentiation degree of cancer cells. X2 test ind ica ted that expression ofEzrin protein and mRNA had no connection with sex, age, sites of metasta tic tumor, andwhether prima ry ma ligna ncy exists (P>0.05), but was releva nt to differentiation degree ofcancer cells (P<0.05). With decrease of differentiation of cancer cells , positive expressionrate of Ezrin protein and mRNA increased.Conclusion1. After bone metastases of multiple ma ligna nt tumors (lung cancer, prosta te cancer,thyroid cancer, liver cancer, etc.), the Ezrin protein and mRNA is still characterized by highexpression, and the expression in metasta tic bone tumors is significa ntly higher than inbenign lesions of bone. Thus it is a promising potentia l marker of metasta tic bone tumor.2.There is a positive correla tion between expression of Ezrin and CD44V6 . It suggeststhat the two ma y pla y a synergistic role in invasion and transfer of metasta tic bone tumors.When the two are both highly expressed, it ind ica tes that the tumor ma y be highlyaggressive and metasta tic, and there ma y be an unfavorable prognosis.3. In the tissues of metasta tic bone tumor, positive expression rate of Ezrin protein and mRNA increase s with decrease of differentiation of cancer cells , but has no connectionwith sex, age, sites of metasta tic tumor, and whether prima ry ma ligna ncy exists . Beca useexpression of Ezrin protein and mRNA is closely related to the differentiation degree ofcancer cells , Ezrin ma y become an ind ex for predicting metasta tic potentia l of metasta ticbone tumors.
