Recombinant human erythropoietin is demonstrably effective in increasing hemoglobin mass, maximum oxygen consumption and physical work capacity. The use of recombinant human erythropoietin is prohibited by the International Olympic Committee. Current tests of recombinant human erythropoietin abuse adopted by international Olympic Committee are urine test and blood test. The blood test is based on an experiment of Recombinant human erythropoietin administration and a statistics result. Someone argued that in some case, such as low hemoglobin baseline, high workload endurance training, low dose of recombinant human erythropoietin administration, the blood test could induce false positive or masking a true positive. It is necessary to carry on further study to improve the blood test.Subjects of this study are ordinary college students who were divided into three groups as follow, high dose of recombinant human erythropoietin administration group, low dose of recombinant human erythropoietin administration group and placebo group. Conclusions are as follow:1. Hct, %Macro, RetHct, serum EPO concentration, serum sTfr concentration were significantly elevated throughout administration. Five parameters of blood test of Low dose group are lower than high dose group. During wash-out, Five parameters of blood test decreased. At the 28* day of wash-out, Hct of administration group was significantly higher than placebo group, both RetHct and serum EPO of administration group were significantly lower than placebo group, serum sTfr concentration returned to baseline level.2. RBC and reticulocyte parameters, including RBC counts, reticulocyte counts, MCVr, RetHb, CHCMr, MCVratio, significantly elevated throughout administration. Serum ferritin concentration of high dose group decreased. Serum ferritin concentration of low dose group didn't change significantly. During wash-out, all ofthese RBC and reticulocyte parameters decreased, Serum ferritin concentration incereased. At the 28th day of wash-out, reticulocyte counts, MCVr, RetHb, CHCMr, MCVratio significantly lower than placebo group, Serum ferritin concentration of all subjects significantly increased, RBC counts significantly higher than placebo group.3. After recombinant human erythropoietin administration VO2max was elevated, Heart rate and blood lactate concentration during submaximal exercise were reduced, showing that the physical work capacity was improved. At the 28th day of wash-out, V02max of administration groups was higher than placebo groups, heart rate and blood lactic acid concentration of administration groups during submaximal exercise had not significant difference with placebo group but significantly higher than baseline, showing that the effects of recombinant human erythropoietin administration may maintain at least 28 days.4. Mask of true positive exists during administration and wash-out. The sensitivity of blood test are less than 10% at the 12th day after end of administration, indicating athletes may increase physical work capacity by recombinant human erythropoietin administration without any punish.5. Recombinant human erythropoietin abuse should not be judged by any upper limit but judged by the unusual changes of blood parameters.
