Background Tobacco use has become one of the most serious public health problems in the world, yet plausibly preventable. The rate of Tobacco use among residents is rising in developing country, but descending in developed country. Also with the obvious up-trend among teenagers, projections indicate that tobacco use do significant harm to adolescent's health. From tobacco onset to tobacco use is a complex process, associated with some factors in personal life including personality, family, school and social factors. Are these factors association with tobacco use of Qingdao students?Objective and Methods In order to explore tobacco use status and related risk factors among middle school students in Qingdao and provide golden rules for intervention measures in smoking control. In this study, a stratified-cluster sampling strategy was used, total 9 junior schools, 9 high schools and 3 occupational schools were sampling in first sampling stage, and then one class from first and second grade in every junior and high school, two classes from first and second grade in every occupational schoolwere chosen in second sampling stage. A total of 2038 students were investigated with questionnaire in the study.Results and Conclusions In the 2038 students, a total of 442 experimental smokers, the average rate of experimental smoking is 21.7% (junor school students' 13.2%, high school students' 28.3% ). A total of 55 smokers were reported. The total rates of smoking are 2.7% (junor school students' 1.8%, high school students' 3.4%). The rates of experimental smoking in male and female students are 28.2% and 15.3% respectively. Male students' rate is evidently higher than female students (X~2=54.175, P<0.001). The rates of smoking in male and female students are 5.2% and 0.5%, and male students' smoking rate is evidently higher than female students (X~2=43.326, P<0.001). Experimental smoking and smoking rate showed obvious up-trend with grade increasing ( X~2=72.103,P<0.001 ; X~2=10.744,P<0.001) ; Both experimental smoking and Smoking rate in occupational school were significant higher than that in general high school (X~2=l 7.434, P<0.001; X~2=27.167, P<0.001).By single-factor analysis, it was suggested that gender, parents' control and supervision, study merit, number of peers and good friends smoking, social conventions and custom, care about appearance, ban of smoking in public were related to experimental smoking. With analysis of Logistic Regression, it is indicated that the most significant factors was gender and number of good friends smoking for Experimental smoking. It is also indicated that the most significant factors are grade and number of good friends smoking for smoking.Above all, the rates of experimental smoking and smoking are low among middle school students in Qingdao, and it was associated with many factors.Suggestions Enhance health education, peer education, induct student's curiosity rightly and smoking control program participated in by family, school and community together were the most effective ways to prevent and reduce middle school students smoking.
