OBJECTIVEColorectal polyp is defined as a growth on the lining of mucous membrane. It can occur at any location along the colon, and display non-specific clinical symptoms like bloody stools, abdominal pain, mucous stool, constipation etc. It is currently accepted that adenomatous polyp is a premalignant condition. The prevalence of colorectal cancer in China is increasing over the years, hence the diagnosis and treatment of colorectal polyp is very significant. There is no consensus on the Traditional Chinese Medicine (TCM) name of colorectal polyp currently, however, it can be considered as "hemafecia", "diarrhea", "abdominal pain", "abdominal mass" etc.TCM body constitution theory is related to colorectal polyp to a certain degree, as the different body constitution will decide the susceptibility of an individual to any given disease. Clinically, there is little study on the relationship between colorectal polyp and TCM body constitution. In this study, we are aimed to conduct an epidemiological study on TCM body constitution in order to understand the distribution of TCM body constitution, and conduct further statistical analysis to identify high risk TCM body constitution type and risk factors for the occurrence of adenomatous polyps, thereby providing some clinical basis for early detection, treatment and prevention of recurrence or progression to malignancy.METHODSWe designed the questionnaire on the basis of extensive literature research and the opinions of experts. Questionnaire:1.Evaluation form of constitution of TCM:This form is based on the Classification and Determination of Constitution of TCM, which is formally released by the Chinese Society of Traditional Chinese Medicine in 2009. We collected cases studies of patients who are diagnosed with colorectal polyps in the Endoscopy Center in Dongfang Hospital, Second Affiliated Hospital of Beijing University of Chinese medicine, and determine the type of constitution they belong to.2. General information and epidemiological data:age, gender, height, weight, body mass index (BMI), smoking history, history of alcohol consumption, medical history, family medical history, mental state, eating habits, defecation problems, etc.3.Endoscopic and pathological diagnosis:the amount, location, type, size, and histological classification of the polyps.4. Establishment of database:analyze the distribution of the patients’constitution of TCM, risk factors of adenomatous polyps and reveal their relevance.RESULTS1.Basic information:A total of 276 cases of patients with colorectal polyps was collected, including 166 male patients and 110 female patients. The average age of the patients is 57.47±11.09 years, the average BMI is 24.73±3.44, reflecting an overweight range. There are 152 cases of adenomatous polyps, and 124 cases of non-adenomatous polyps.137 cases are found with single polyps and 139 cases with multiple polyps, both about 50% each. 2.Endoscopy results:Colorectal polyps are commonly discovered in the sigmoid colon, transverse colon and rectum. Polyp size in d<0.5cm,0.5cm<d<1.0cm is the most common. Age, the amount of polyps, the type of the polyps and defecation problems are significantly related with adenomatous polyps. Older people, multiple polyps, type III, type IV are more prevalent in adenomatous polyps.3.Constitution of TCM results:The distribution of the patients’constitution of TCM:Phlegm-dampness constitution (27.2%)>Dampness-heat constitution (25.9%)>Yang-insufficiency constitution (24.1%)>Qi-stagnation constitution (17.2%)>mild physical constitution (16.3%)>Yin-insufficiency constitution (12.9%)> qi-insufficiency constitution (12.5%)> Blood-Stagnant constitution (8.2%)>specific endowment constitution (0.4%). Phlegm-dampness constitution, Dampness-heat constitution and Yang-insufficiency constitution make up 77.2%of the whole sample. The constitution distribution between adenomatous polyps and non-adenomatous polyps is basically the same. Through multivariate logistic regression analysis of factors relating with adenomatous polyps, the factors are the number of polyps> defecation problems>gender.CONCLUSION1. The main types of constitution of TCM in colorectal polyps patients are Phlegm-dampness constitution, Dampness-heat constitution and Yang-insufficiency constitution. There is no difference between the constitution distribution of adenomatous polyps and non-adenomatous polyps patients.2. The risk factors of adenomatous polyps are gender, the number of polyps, the histological classification and defecation problems.
