Purpose:Provide an objective basis of biomedical research for the study of the specificity of the effect that infrared temperature changes of Bulang reflected the pathological changes of chronic HBV infection. Explore the status of chronic HBV infection, characteristics of sensitization induced by the kidney-meridian acupoint Bulang and the pathologic mechanism of intrahepatic venous blood perfusion through a comparative study between patients with chronic HBV infection( chronic hepatitis B,compensatory period of chronic hepatitis B cirrhosis) and healthy volunteers in following aspects: liver’s anatomic projection area and different acupoints in contralateral region,meridian points and other areas on the skin,infrared surface temperature,and hepatic vein and portal vein hemodynamic parameters.Object and method:In this study there were two groups. One is disease group that consists of 48 patients with chronic HBV infection from Infectious disease hospital of Benxi City, the other is health group that consists of 44 healthy volunteers from Liaoning University of Traditional Chinese Medicine.This study used medical ATIR-M301 infrared thermal imager to observe the subjects.Collected all over body positive infrared image fist,then selected points for measuring temperature from the liver surface projection region in infrared image and the offside( Qimen LR14,Riyue GB24,Rugen ST18,Bulang KI22,Shidou SP17),non-acupoint( Ourter-Qimen,Inter-Qimen,Ourter-Riyue,Inter-Riyue) and recoder temperature data of each temperature measuring point. This study used Tanzhong CV17 which has the same temperature with liver exposed as the basis temperature. Calculated infrared relative temperature( Temperature measuring point/Tanzhong CV17),then recorded and collect data.This study also used the GE730 color Doppler ultrasonic diagnostic instrument which made in the United States with a 3.5 MHz-frequency convex array probe. Measured and recorded the portal vein diameter of DPV( D) and maximum blood flow velocity of blood vessel VPV( V) in hepatic portal vein branches before 1 cm. Then calculated congestive index of portal vein( CI),CI=π×( D/2)2/v. We judge it as portal hypertension if CI > 0.08,and the results were divided into the chronic hepatitis B group and the portal hypertension group( high pressure group). Measured and recorded the diameter of middle vein DHV( D),Velocity of blood flow VHV( V) and Blood flow spectrum HVF.Explore infrared temperature change and its characteristics of the liver in different health state by infrared relative temperature pairing between groups of healthy group,chronic hepatitis B group and high pressure group. Explore wave pattern change in different health condition by hepatic venous wave( HV0、HV1、HV2)frequency pairing between groups of health group, chronic hepatitis B group and high pressure group. Explore infrared temperature variation in Bulang and the pathological mechanism of specific diseases and blood perfusion of hepatic vein by studying the relationship of infrared temperature changes in healthy group and disease group with DHV, VHV, HVF, DPV, VPV, and CI.All data were analyzed using statistical analysis software SPSS17.0. And we make group matching test by NPar test and Mann-Whitney test. The mean is expressed as`c±s. Statistical result was statistically significant if P≤0.05 and was no statistical significance if P > 0.05.Correlation analysis using Spearman correlation coefficient of the two variables rank correlation to judge,they are positive correlation if r > 0,negative correlation if r <0,correlation if r=0. The value of r was significantly correlated with the degree of correlation,there is significant correlation if P ≤0.05,there is no significant correlation if P>0.05.Results:1. Infrared temperature variation of liver meridian in different liver health status.1)Chronic hepatitis B group showed a trend of decreasing temperature when contrast between chronic hepatitis B group and health group,except Bulang R. Bulang R,Qimen R,Riyue R,Inter-Qimen R, and Inter-Qimen L all have statistical significance(P>0.05).2)Pressure group showed a trend of decreasing temperature when contrast between portal hypertension group and health group. Bulang R,Qimen R,Riyue R,Inter-Qimen R,Inter-Qimen L and Inter-Riyue L all have statistical significance(P<0.05).3)Pressure group showed a trend of decreasing temperaturewhen contrast between portal hypertension group and chronic hepatitis B group. Bulang R,Qimen R,Riyue R,Inter-Qimen R,Inter-Qimen L and Inter-Riyue L all have statistical significance(P<0.05).2. Correlation analysis of hepatic vein and portal vein hemodynamics and liver surface in the infrared temperature.1)The relative temperature change of Bulang R in disease group was negatively correlated with DHV(P<0.05). And it was negative correlation with VHV(P>0.05). Qimen R,Riyue R,Inter-Qimen R,Inter-Qimen L and Inter-Riyue L in disease group compared with DHV and VHV all have no statistical significance(P>0.05). There was no statistical significance in the correlation analysis of health group(P>0.05).2)The relative temperature change of Bulang R in disease group was positive correlation with VPV(P<0.05),and it was negative correlation with CI(P<0.05),negative correlation with DPV(P>0.05). Qimen R and Inter-Qimen R in disease group compared with DVP and CI were negative correlation(P<0.05),and compared with VPV were positive correlation(P<0.05). There was no statistical significance in the correlation analysis of health group(P>0.05).3. Correlation analysis of infrared temperature of liver’s anatomic projection area and hepatic vein wave pattern.1)HVF changes of liver in different health status.(1)Compared with health group,HV0 in Chronic hepatitis B group showed a decreasing trend,but HV1 and HV2 showed a increasing trend(P<0.05).(2)Compared with health group,HV0 in high pressure group showed a decreasing trend,but HV1 and HV2 showed a increasing trend(P<0.05).(3)Compared with Chronic hepatitis B group,HV0 in high pressure group showed a decreasing trend,but HV1 and HV2 showed a increasing trend(P>0.05).2)Correlation analysis of liver’s anatomic projection area infrared temperature and HVF: the change of infrared temperature of Bulang R was negative correlation with HV1(P<0.05).And Qimen R,Riyue R,Inter-Qimen R,Inter-Qimen L,Inter-Riyue L were negative correlation with HV1(P>0.05).Conclusion:1. Effect of infrared temperature change in Bulang reflecting the pathological changes of chronic HBV infection based on the changes of hemodynamics of hepatic vein. It provides a valuable reference for the research of liver and kidney homology, conversion of jing and blood, liver and kidney deficiency etc.2. Through the differences of the pathological changes of the hepatic venous system were reflected by the infrared temperature changes of Bulang,Qimen,Riyue. It illustrates that is necessity to study the specificity of acupoints.
