Clinical Study on Effect of Zhixinkang Capsule in Treating Unstable Effort Angina and Protecting Vascular Endothelium Specialty: The Study of Cardiovascular-Diseases, Internal Medicine of Integrated Traditional and Western Medicine Graduate : Gao Fujun Tutor : Prof. Zhang Wengao Abstract Objective To observe the clinical effect of Zhixinkang Capsule (ZXKC) on unstable effort angina and study its therapeutic mechanism. Methods Sixty-five patients of coronary heart disease (CHD) with unstable effort angina were divided randomly into ZXKC group (34 cases) and control group (31 cases). Conventional western medical therapy was given to both groups and the ZXKC group received additional ZXKC treatment. The changes of clinical symptoms and laboratory indexes in all the patients were observed before and after treatment. Data of 20 healthy persons were taken as normal group. Results The peripheral circulating endothelial cells (CEC), the platelet P-selectin (CD62p), the plasma endothelin (ET) and the malonyl- dialdehyde (MDA) levels of all the patients increased while the serum nitric oxide ( NO) and the superoxide dismutase ( SOD) levels of the patients decreased; PFVA, A/E rose and PFVE, SV, CO lowered. These chan~ges above were statistically different compared with those of the normal group, P<0.05. After treatment with the ZXKC, the total effective rate of relieving angina, improving ECG and meliorating symptoms were respectively 97.06%, 82.35% and 100%; the corresponding remarkable rate were respectively 76.47%, 35 .29% and 88.24%, which were significantly better than those of the control group, P<0.05~棐0.01. Meanwhile, CEC, CD62p, ET, MDA decreased and NO, SOD increased; PFVE, SV, CO rose and PFVA, A/E lowered, which were statistically different compared with those of the control group, P<0.05----- 0.01. Conclusions There are severe damage of vascular endothelium, elevation of the degree of platelet activation, aggravation of lipid peroxidation and decline of the systolic and diastolic function of heart in the patients. The ZXKC could effectively resist myocardial ischemia and relieve angina. Its therapeutic mechanism for unstable effort angina might be related with resisting lipid peroxidation, protecting vascular endothelium, inhibiting the activation of platelets, regulating lipid metabolism, reducing myocardial oxygen consumption and improving function of heart.
