Preventive Effect Of THP And MMC For Post-operative Superficial Bladder Cancer By Instillation

Objective: To compare the effect and safety of Pirarubicin(THP) and Mitomycin (MMC) by bladder instillation after transurethral resection of superficial bladder cancer,and investigate their clinical significance.Methods: 97 patients with superficial bladder cancer after transurethral resection were selected and divived into two groups. 51 cases received intravesical instillation of Pirarubicin (THP group); 46 cases received intravesical instillation of Mitomycin (MMC group). All the patients were advised not to drink before intravesical instillation, laid low after rowing over urine. Put catheter into bladder after disinfection,then slowly push the perfusion fluid . THP group kept drug 30 minutes, MMC group kept drug 2 hours, changed the position every 10 minutes( left, right, supine, prone lie down).Once a week bladder instillation was performed on all patients 7 or 10 days after the operation. 8 weeks later once a month for 1 year. Observated the adverse reaction and cystoscopy was performed regularly to compare with the tumor recurrent rates.Results: All patients were follow-up for 24 months. The tumor recurrent rates were 5.88%,19.57% in THP,MMC groups after one year,espectively;The group The tumor recurrent rates were 11.76%,28.26% in THP,MMC groups after two year,espectively. There are significant difference between THP and MMC group(P<0.05). The incidence of urethra irritation were 13.73%, 32.61% in THP,MMC groups, espectively; The incidence of haematuria were 9.81%,26.09% in THP,MMCgroups, espectively. There are significant difference between THP and MMC groups(P<0.05).Conclusion: THP would be more effective than MMC in prevention of recurrence of superficial bladder cancer. The side effects were lower using by THP than MMC, and the tolerance and compliance were better.THP was worthy using in clinical.
