In order to provide information for teaching, improve children's school readiness, avoid to produce school adjustment problems, the objective of this study is to produce a Parent's Questionnaire of Chidren's School Readiness.The questionnaire focuses on first-year students,one of children's caregiver fills in the questionnaire,it may take up about 10 to 15 minutes. It divides into two parts:(1)Children's demographic information,just like nature information about theirselves,family structure, main caregiver,parents'education degree and professional,fill across the facts;(2)The formal test part,it includes 49 items, divided into three subtests: development risks, type of temperament and self control test. The development risks test includes three dimensions, externalization problems, internalize problems and learning difficulties. Type of temperament test includes two dimensions, withdraw and approach. Self control test includes two dimensions, behavioral and emotional regulation. The answer to the questions is"Yes"or"No",except open-ended question .The questionnaire has good reliability and internal consistency. The dimensions of the items were above 0.3, and the average discrimination of the sub-tests were more than 0.4; the internal consistency of each dimension of all the three sub-test questionnaire ranged from 0.577 to 0.797. The revised structure of the questionnaire model subtests meet the the basic indicators.The scores of the questionnaire can predicate the future development of the children. The scores of development risk subtest were able to predict the potential external, internal prob- lems and academic difficulties; the scores of self-control sub-test were able to predict the children's external problems ; the scores of cognitive and behavioral self-control sub-test and total score can predict academic difficulties.According to the sub-test scores, the externali-zation problems risk, internalizing problems risk and academic difficulties were divided into low risk, middle level and high-risk, the ability to regulate cognition and behavior, emotions and needs regulation and general self-control divided into low, middle level and high level. Temperament can be subdivided into withdraw and approach, normal type and special type.According to the evaluation results, teachers will understand children's school readiness, get the information about children's potential development risks, the level of self-regulation and children's temperament types, which provide some information for the teaching of that in accordance of their aptitude. Teachers will take appreciate strategies on the bases of children's actual development situation,then foster strengths and circumvent weaknesses. It will prevent their school maladjustment, make their progress.
