Objective To investigate the clinical significance of interleukin-18,12and tumor necrosis factor-α in idiopathic hypercholanaemia of pregnancy.Methods70cases of pregnant women with GlycocholicAcid≥10.75μmol/L, including40cases of dysfunction of liver (ICP group),30cases of normal liver function (19cases were asymptomaticgroup,11cases were symptomatic), and30cases of normal pregnant women(control group) were recruited. Serum IL-18, IL-12and TNF-α were examined by ELISA. Serum alanine aminotransferase (ALT) and aspartateaminotransferase (AST) were examined by automatic biochemical analysisinstrument.Results (1) In ICP group,serum concentrations of IL-18, IL-12andTNF-α were (129.6±22.9)ng/L,(76.3±21.4)ng/L and (84.5±12.3)ng/L. In normal liver function group, serum concentrations of IL-18, IL-12and TNF-α were(67.8±15.2)ng/L,(31.6±10.2)ng/L and(51.4±10.7)ng/L. While in control group, serum concentrations of IL-18, IL-12andTNF-α were(43.4±13.2)ng/L,(10.2±3.4)ng/L and (32.5±9.0)ng/L.Serum IL-18, IL-12and TNF-α levels in hypercholanaemia of pregnancygroup were significantly higher than those in control group(P<0.05).SerumIL-18, IL-12and TNF-α levels in ICP group were significantly higher thanthose in normal liver function group(P <0.05).In normal liver functiongroup, serum IL-18, IL-12and TNF-α levels between pruritus group and nopruritus group showed no significant difference(P>0.05).(2) Theincidences of premature birth(60%) and meconium-stained amniotic fluid(30%) in dysfunction of liver group were significantly higher than thosein normal liver function group (23%,7%) and normal controlgroup(P<0.05).The incidences of premature birth(23%) andmeconium-stained amniotic fluid in normal liver function group weresignificantly higher than those in normal control group(P<0.05). While innormal liver function group, The incidences of premature birth between pruritus group (11%) and no pruritus group (27%) showed significantdifference (P <0.05).Conclusion The expressions of IL-18, IL-12and TNF-α betweenidiopathic hypercholanaemia of pregnancy and intrahepatic cholestasis ofpregnancy is different. The maternal and neonatal outcomes inhypercholanaemia of pregnancy are good. Objective To investigate the effect of interleukin-18and interleukin-12in hepatic injury induced in intrahepatic cholestasis of pregnancy(ICP).Methods Six-two cases of ICP patients(ICP group),30cases ofhealthy pregnant women (control group) and30cases of hepatitis B(HBV)women (hepatitis group) were recruited. Serum IL-18,IL-12and TNF-αwere examined by ELISA. Serum alanine aminotransferase (ALT) andaspartate aminotransferase(AST) were examined by automatic biochemicalanalysis instrument.Results (1) The levels of IL-18, IL-12, TNF-α, AST and ALT in hepatitis group were significantly higher than those in ICP group andnormal control group,the difference was statistically significant (P <0.05);The levels of IL-18, IL-12, TNF-α, AST and ALT in ICP group weresignificantly higher than those in normal control group, the difference wasstatistically significant (P <0.05).(2) The levels of IL-18, IL-12, TNF-α,AST and ALT in severe ICP group were significantly higher than those inmild ICP group and normal control group, the difference was statisticallysignificant (P <0.05); The levels of AST and ALT in mild ICP group wassignificantly higher than those in normal control group, the difference wasstatistically significant (P <0.05); compared with the levels of IL-18andIL-12in mild ICP group and normal control group,there was no statisticallysignificant difference (P>0.05).(3) The morbidity of premature infant andMeconium-stained amniotic fluid in the severe ICP group weresignificantly higher than those in mild ICP group and normal control group,the difference was statistically significant (P <0.05).Conclusion IL-18may be involved in process of hepatic injury ofICP.
