Eczema (eczema) is a relatively common delayed allergy, allergic inflammatoryskin disease. Clinical often symmetrical distribution, polymorphous damage, severeitching, wet tendency, repeated episodes of chronic performance, easy. Eczema is ahigh incidence, which accounts for outpatients in the Department of Dermatology of15~25%.A new study suggests the United States of America, various types of eczemaprevalence rate was18%in the general population; and a number of research resultsin England found that all patients with skin disease eczema patients accounted for17-30%. For children, eczema is a common allergic diseases, and with theimprovement of living standards and changes in climate and environment, in recentyears the incidence rate is rising. Because the eczema is a kind of allergic disease, itsetiology is complex, often by a variety of endogenous and exogenous factors (or) lead,many allergens cause eczema, exogenous common are: chemicals, dyes, cosmetics,some foods (such as eggs, fish, shrimp, milk) and some pollen and dust mites;endogenous common are: in vivo infection, intestinal parasitic disease, endocrine anddigestive function disorder. The pathogenic pathways often for type I or type IVallergic reaction of body exposure to allergens, the latter for the allergen directly intothe body of a patient, IgE adsorbed on the surface of the mast cells, mast cells releasea variety of inflammatory mediators and cytokines, cause skin allergy. Psoriasis(psoriasis) of chronic inflammatory skin disease is a common childhood, close contactwith allergic factors. Psoriasis etiology and pathogenesis are not clear, it is usuallyconsidered to be a complex and genetic correlation of autoimmune disease. With theprogress of the disease, psoriasis can change several significant tissue:(1) the rapidproliferation of keratinocytes, thickening of the epidermis (acanthosis).(2) abnormalkeratinocyte differentiation, the granular layer decreased or disappeared (granularlayer and stratum corneum cells have reduced) nuclear residue (dyskeratosis).(3)dermal papillary vascular expansion leads to visible erythema.(4) the dermis appeareddense infiltration of inflammatory cells.The organization of psoriasis pathology are chronic inflammatory diseases by means of inflammatory mediators and cytokinesmediated, maintain and amplify. It has been suggested that IgE and eosinophilcationic protein (ECP) is the pathogenic factors of psoriasis inflammation in thenetwork, its synthesis and secretion might be regulated by other inflammatorymediators or cytokines in patients with psoriasis, dermal inflammation of the skin siteincreased mast cells. Similarly, the IgE molecular allergen enters the body and mastcell surface binding, and then to mast cells release a variety of inflammatorymediators and cytokines, cause and aggravate skin allergic reaction.Eczema and psoriasis is a common allergic skin disease, IgE and IgG4are themain antibody mediated allergic reaction. Most allergic reaction mediated by IgE forimmediate, it and histamine plays an important role in IgE mediated I type allergicreaction, currently has almost become a sign of success indicators to evaluate foodsensitization animal model. Therefore, detection of food specific IgE antibody is aneffective method to judge the allergic skin disease. Hypersensitivity reaction mediatedby IgG4is delayed, from eating certain foods, and the lack of relevant digestiveenzymes which could not be completely absorbed, will produce food specific IgG4antibody, blocking the renal filtration structure, not the normal excretion and retentionin the relevant tissues, produce a series of allergic symptoms and diseases. Results thedoctoral dissertation of Huang Qiong shows the content of serum IgG4and IgEantibodies and histamine that food miniature pig sensitized model can becomeavailable model allergy evaluation in strong correlation of each individual, each timepoint. This indicated that IgG4and IgE can be used as effective indicators of foodallergic diseases. Previous domestic studies have also shown that correlation existsbetween psoriasis and eczema, allergic factors, specific IgG4, IgE allergy testing canfast, convenient to guide clinical treatment, and help patients to eat right.Given the close relationship between detection of IgE and IgG4antibodies ineczema and psoriasis diagnosis, this paper will investigate the common analysis ofHangzhou area children eczema and psoriasis two allergic skin allergens and relatedfactors, to explore the application value of serum IgE and IgG4levels detection inchildren with eczema and psoriasis diagnosis, and cause analysis, analysis of the relationship between food specific IgG4antibody in children with eczema andpsoriasis, analyze its correlation with the cell factor, and analyze the relatedmechanism, which will provide definitive etiology and pathogenesis of eczema andpsoriasis and other allergic skin disease, so as to determine the this kind of diseaseprevention, diagnosis, allergen and targeted radical provide certain theoretical basis。Objective: to clear the cause of children eczema and psoriasis main food andinhalant allergens, explore the significance of detection of serum specific IgG4antibodies and IgE antibodies in patients with allergic skin disease diagnosis andprevention; at the same time the child feeding and eczema and psoriasis, provide thebasis for this kind of disease prevention and treatment.Method: choose the inclusion criteria of children with eczema and psoriasis,14kinds of food allergens and5kinds of inhaled allergens, using enzyme-linkedimmunosorbent assay (ELISA) determination of the exogenous specific IgE and IgG4antibody in serum.At the same time to investigate the relationship of children fed witheczema and psoriasis.Results:60patients met the inclusion criteria of eczema in children with20detection of the exogenous allergen-specific IgG4antibody positive rate is higher formilk (46.70%), eggs (33.3%), soybean (10%), rice (10%), shrimp (10%), ragweed(10%);60cases with selected standard psoriasis children20detection of theexogenous allergen-specific IgG antibody positive rate is higher for milk (40.00%),eggs (36.7%), soybean (11.7%), shrimp (10%), ragweed (10%),beef(6.7%).And foodallergen-specific IgG4antibody and IgE antibody positive rate was no significantdifference; the positive rate of inhaled allergen specific IgG4antibody weresignificantly higher than those of IgE antibodies.Food allergen specific IgG4positiverate was higher than that of the inhaled allergen positive rate of specific IgG4.Thepositive detection rate of eczema patients all kinds of allergen IgE and IgG4: thepositive rate of specific IgG4eggs and rice were33.3%and20%, the positive ratewas significantly higher than that of specific IgE (P <0.05); the positive rate of fish,shrimp, crab and short ragweed specificity were13.3%, Ig E33.3%,16.7%,25%, thepositive rate was significantly higher than that of specific IgG4(P <0.05), no significant differences in the other13kinds of allergens positive rate of antibody (P>0.05); the positive detection rate of psoriasis patients all kinds of allergen IgE and IgG:the positive rate of specific IgG4eggs and rice respectively.33.3%and20%, thepositive rate was significantly higher than that of specific IgE (P <0.05); the positiverate of specific IgE fish, shrimp, crab and short ragweed was respectively13.3%,33.3%,16.7%,16.7%, the positive rate was significantly higher than that of specificIgG (P <0.05), no significant difference between the other13kinds of allergenspositive rate of antibody (P>0.05).Conclusion: milk, egg, soy, rice, shrimp, short ragweed easily lead to eczema,milk, egg, soy, rice, shrimp, short ragweed and beef easily cause psoriasis.SpecificIgG4, IgE antibodies to quickly, effectively find allergen, and effectively guide theclinical prevention and treatment; children with IgG4and IgE antibody mediated foodand inhalant allergy skin disease and feeding method in the early stage after birth,artificial feeding and mixed feeding children more eczema and psoriasis and otherallergic skin disease.
