Objective: The first, to explore the expressions of vasoactiveintestinal peptide (VIP) and tumor necrosis factor-α (TNF-α) in expressedprostatic secretions(EPS) of patients with type Ⅲ prostatitis. The secondto discuss the correlation of VIP、 TNF-α and NIH-chronic prostatitissymptom index(NIH-CPSI)、international prostate symptom score(IPSS)、tenderness of CPSI. The third, to discuss the Effects of VIP, TNF alpha inmechanism of type Ⅲ in thepathology process of prostatitis.Method: Sixty-nine patients with type Ⅲ prostatitis and thirteennormal young men were investigated including37cases of type Ⅲa and32cases of type Ⅲb. Medical records were established, and the patientswere investigated with NIH-CPSI, IPSS, tenderness score of CPSI.Sampling the EPS form patients with Ⅲ type prostatitis and normalyoung men, levels of VIP and TNF-α were measured by two-antibodyenzyme-linked immunosorbent assay. Analysis the correlation of thetesting results of VIP, TNF alpha and the NIH-CPSI, IPSS and tendernessscore of CPSI.Result: Contents of VIP in EPS were significantly higher in typeIIIa and type IIIb groups (Ⅲa group:(51.33±10.63) pg/ml; Ⅲb group:(47.45±7.55) pg/ml) than in control group ((25.95±4.74)pg/ml).Contents of TNF-α were significantly higher in type Ⅲa group((76.08±15.44)pg/ml) than in type IIIb group ((27.85±6.43) pg/ml) andcontrol group ((10.30±3.62) pg/ml); significantly higher in IIIb groupthan in control group with statistical differences (P<0.001). Contents ofVIP were positively related with NIH-CPSI scores(r=0.711,P<0.001)and IPSS scores(r=0.428,P<0.001)and tenderness score of CPSI(r=0.517,p<0.001). Contents of TNF-α were not significantly relatedwith NIH-CPSI scores, IPSS scores and tenderness score of CPSI(p>0.05).Conclusion: The first: contents of VIP and TNF-α were significantlyhigher in type Ⅲ prostatitis groups than in control group, prompting thatthe VIP and TNF-α may play an important role in the process of type IIIprostatitis. The second: differences were existed in TNF-α contentsbetween type Ⅲa group and type IIIb, prompting that there may bedifferences in etiology and pathogenesis of type Ⅲa and type IIIbprostatitis. The third: the contents of VIP are significantly higher in typeⅢ prostatitis groups than in control group, and contents of VIP werepositively related with NIH-CPSI scores and IPSS scores and tendernessscore of CPSI, prompting that detection of VIP and may provide valuableindicators in the diagnosis and treatment of type Ⅲ prostatitis and help toexplore the mechanism of neural endocrine factors in the process of typeⅢprostatitis.
