Because of the safe operation of urban water supply system, not only the water with both quality and quantity can be provided to urban residents, to ensure the healthy of the public, also the development of urban economy can be promoted, the social stability can also be maintained. In recent years, with the continuous development of city and the improvement of water supply system, problems,the assurance of water resources was dying sown, water pollution was serious, water infrastructure was developed imbalance, leakage phenomenon was serious, the capacity of water quality monitoring was weak, and emergency capacity was weak, were appeared, urban water system was challenged again. Therefore, profound significance will be given to build perfect urban water supply system by comprehensive evaluation of urban water supply system and finding out the weakness of water supply security.Fuzzy synthetic evaluation model is taken as basic in this research, in the process of in-depth understanding the fundamentals of fuzzy evaluation model, the quantity of evaluation indexes and the method of indexes weight determining were the important factors affecting the evaluation results in the process of comprehensive evaluation, were found. To solve these problems, the preliminary indicator set was analyzed through information difference analysis and information redundancy analysis, those no significant difference or weak significant difference indexes and those inflecting information repeated indexes were deleted, 12 indexes were screened to construct the evaluation index system of Zhoukou city. In addition, indexes weight set were determined by means of the Improved Analytic Hierarchy Process(IAHP) and Entropy Evaluation method, in order to avoid the difference between practical importance with the weight determined by single method, the weights determined by two methods were weighted average to make up for the inadequacy of single weight determination method as far as possible.Relying on the “12th Five-Year” national water pollution control and governance of major science and technology projects, “ the water district of South to North Water District drinking water safety technology research and comprehensive demonstration”(NO. 2012ZX07404-004) topic, taking Zhoukou city as the research object, the safety grade of the water supply system in 2005~2014 and long term were calculated by using the multilevel fuzzy evaluation model, the existing problems were analyzed and the corresponding suggestions were put forward. The main conclusions were summarized below:Water supply security levels in Zhoukou city from 2005 to 2014 have been gradually promoted, however, the majority were in the basic safety level, and individual years were even in the dangerous level. Therefor, the water supply security level of Zhoukou were on the low side totality.The the water supply security level of Zhoukou in 2020 were a little bit safer, increased significantly. There were only 16.67% indexes in a low security level. However, Per capita water resources, Network density, and Water consumption per capita daily life were still in a low level, which illustrated there needs to find new water sources, or Improve the water resources situation in the area; pay attention to the construction progress of the network; and improve the residents’ consciousness of environmental protection and water saving.
