Chronic prostatitis commonly occurs in young and middle-aged men, which brings patients a lot of physical pain and mental burden.33%of urology patients suffer from CP. National Institutes of Health declared CP, myocardial infarction, unstable angina, and active Crohn’s disease are the most serious chronic diseases that affect residents’ quality of life. However, the etiology and pathogenesis of CP is not fully clear. A large number of studies have shown that a variety of cytokines may be the leading cause of the CP, and inflammatory response is regulated by the SOCS family of proteins whose role in the process of the disease has not been demonstrated so far.Previous trial have confirmed that Qian Lie Hua Zhuo decoction can significantly improve the symptoms and the quality of life of patients.On the basis of experimental autoimmune prostatitis(EAP) rats, we quantitate the expression of cytokines and SOCS of rats in each group(normal group, model group,and different Qian Lie Hua Zhuo Decoction groups). The aim of our study is to investigate how the SOCS affect the course of the CP and reveal the mechanism of Qian Lie Hua Zhuo decoction treating CP.BPH is often accompanied by inflammation. Inflammation may contribute to the progression of BPH. Except the influence of age ang androgen, Inflammation may be the most important factor leading to the progression of BPH.The man with CP is more likely to suffer from BPH than the man without CP.However,the subjects of previous studies on the correlation between the CP and BPH are BPH patients. After prostatectomy,researcher exert pathological examination on acquired prostate so as to study the correlation between the CP and BPH. Some researchers think this study method belongs to retrospective study. Thus,we can not tell which of both occurs earlier. Age,a factor in BPH progression, could not be excluded either.So, the correlation between the CP and BPH is unclear. There is still no reliable evidence which suggests that the role of CP in the pathogenesis and progression of BPH. Some scholars hold the idea that CP is a kind of autoimmune diseases.On the basis of EAP rat, we reveal the mechanism of CP in the progression of BPH by prospective study. This method does not only make sure CP occurs earlier than BPH but also excludes the effect of age on BPH(age can not influence the BPH of rat). This helps to reveal the role of CP in the progression of BPH and the mechanism of Qian Lie Hua Zhuo decoction blocking the BPH progression.Some scholars think the prevention and treatment of CP could block the progression of BPH.Thus, futher study on cytokines will be needed which may provide a potential therapeutic targets to delay BPH initiation and progression. On the basis of EAP rat, We examine interleukin-1(IL-1),interleukin-6(IL-6), interleukin-10(IL-10), interleukin-17(IL-17), tumor necrosis factor-a (TNF-a), Bcl-2, SOCS-1, SOCS-3and pathomorphology in order to reveal the role of inflammation in the progression of BPH and delay BPH initiation and progression.This study is divided into two parts:literature review, and animal experimental research.1Literature Review: In this part, there are two reviews. One is the research advancement on the correlation between CP and cytokines, the other is on the role of CP in the progression of BPH.2Animal experimental research:Study on the Effects of Chronic Prostatitis on Benigh Prostatic Hyperplasia and the Intervention of Qian Lie Hua Zhuo Decoction Objective: To observe the mechanism of Qian Lie Hua Zhuo decoction treating CP and blocking the progression of BPH.Methods:The animal model of experimental autoimmune prostatitis was established by injecting subcutaneously1mL emulsified antigen, the rats’prostate protein in concentrations of15g/L combined with complete Freund’s adjuvant (CFA), at abdominal wall multi-loci and by intraperitoneal injection of pertussis-diphtheria-tetanus vaccine0.5ml, i.P at the0,7,14and21th day.66male Sprague Dawley rats of240-300g were divided into normal group (n=12) and chronic prostatitis group (n=54).48rats of CP were divided into model group, and different Qian Lie Hua Zhuo Decoction groups (in low, middle or high does respectively,12per each group).After 30days’treatment, the rats were sacrificed and prostatic tissues were taken for the pathological examination. Meanwhile, we evaluate the expression of IL-1, IL-6,IL-10, IL-17, TNF-a,and Bcl-2by RT-PCR, quantitating the levels of SOCS-1and SOCS-3by Western-blot.Result(1) The result of pathological examination:EAP was confirmed by pathological examination. Qian Lie Hua Zhuo decoction is effective in treating CP and delaying the BPH progression.(2)The result of inflammatory cytokines:Compared with normal group, interlukin10(IL-10) expression significantly (p<0.01) decreased, and proinflammatory cytokines(IL-1,IL-6, IL-10, and TNF-a) were higher(p<0.01) in model group.Compared with model group, IL-6, IL-17and TNF-a expression levels were lower(p<0.01) in Qian Lie Hua Zhuo Decoction groups (in low, middle or high does respectively),especially in Qian Lie Hua Zhuo decoction middle does group.Compared with model group, IL-1expression level was lower(p<0.01) in Qian Lie Hua Zhuo Decoction middle does group.However, IL-1expression level was not significantly decreased (P>0.05) in Qian Lie Hua Zhuo Decoction low and high does group.Compared with model group, IL-10expression level significantly (p<0.01) increased in Qian Lie Hua Zhuo Decoction groups (in low, middle or high does respectively),especially in low and middle does group.(3)The result of anti-death oncoprotein Bcl-2:The Bcl-2expression level significantly (p<0.01) increased in model group compared to normal group. Compared with model group, The Bcl-2expression level significantly decreased in Qian Lie Hua Zhuo decoction middle does group.(4) The result of SOCS-1and SOCS-3:Compared with normal group, both SOCS-1and SOCS-3expression level significantly were higher(p<0.01) in model group. Compared with model group, both SOCS-1and SOCS-3expression level were lower(p<0.01) in Qian Lie Hua Zhuo Decoction groups (in low, middle or high does respectively),especially in Qian Lie Hua Zhuo Decoction middle does group.ConclusionEAP was confirmed by pathological examination and cytokines expression.After treatment,the effect of Qian Lie Hua Zhuo decoction on CP was also confirmed by pathological examination and the result of cytokines.After30days, compared with the normal group, the expression level of proinflammatory cytokines significantly increased in model group.Moreover, proinflammatory cytokines (IL-1,IL-6, IL-17, TNF-a) significantly decreased in treatment group. Qian Lie Hua Zhuo decoction treats CP by down-regulating proinflammatory cytokines.IL-10expression level significantly decreased in model group. Moreover, it significantly increased in treatment group. Qian Lie Hua Zhuo decoction treats CP by up-regulating IL-10.Compared with the normal group, the expression level of proinflammatory cytokines (IL-1,IL-6,IL-17,TNF-a),SOCS-1, and SOCS-3significantly increased in model group. Both cytokines and SOCS affect the course and progression of CP. JAK/STAT may play an important role in the progression of CP, which need further research.Bcl-2expression level significantly increased in model group and decreased in Qian Lie Hua Zhuo decoction middle does group. The result of pathological examination is that prostate gland occurs papillary hyperplasia in model group and that rarely occurs papillary hyperplasia in treatment group. CP may play a critical role in the progression of BPH, we concluded that Qian Lie Hua Zhuo decoction may delay BPH progression by down-regulating Bcl-2...
