Clinical Research On Treatment Of Polycystic Ovary Syndrome With Combination Of Chinese Traditional And Western Medicine

Objective: In this research,we observe the clinical effects of compound Xuan Ju capsule combined Diane-35 in patients with polycystic ovary syndrome(PCOS) of kidney Yang deficiency to investigate the advantage of combining traditional Chinese and western medicine in the treatment of PCOS.Methods: 62 cases with PCOS of kidney yang deficiency were recruited from reproductive medicine center at Wuhan Integrated TCM and Western Medicine Hospital from June in 2013 to April in 2014.One half of the patients were randomly selected as treatment group,treated with Diane-35 and compound Xuan Ju capsule on menstrual cycle of the 5th day or progesterone withdrawal bleeding of the 5th day,took Diane-35 1 pill per day for 21 days and started the next cycle at the 8th day of drug withdrawl;constantly took compound Xuan Ju capsule 3 times per day and 3 pills at a time despite during the menstrual period.The other half of the patients as control group, simply took Diane-35.After treatment of three menstrual cycles, observed the two groups of patients before and after treatment with TCM symptoms, menstruation, hairy, acne, sex hormone levels(T, E2, LH, FSH, DHEAS, LH/FSH),glucose metabolism targets(FPG,FINS, HOMA-IR),ovarian volume, the number of antralfollicles and evaluated the safty,observed the menstual cycle,TCM symptoms within three months of drug withdrawl.Results: One case was rejected in the treatment group and another case was abscent from the control group,so the actual observation number of each group were 30 cases.①TCM syndrome: there were 12 cases clinical recovery,12 cases markedly effective,3 cases effective in the treatment group of 30 cases,so the total effective rate was 90.00%;the control group included 30 patients,among whom 5 cases were clinical recovery, 6 cases were markedly effective, 8 cases were effective, thus the total effective rate was 63.33%.The TCM syndrome of two groups by Ridit analysis is P<0.05, so the difference has statistical significance.②The improvement of TCM syndrome integral:compared with before treatment the group after treatment as well as three months’ discontinuation, the TCM syndrome integral declined obsviously and the difference was statistically significant(P < 0.05);After the treatment and 3 months’ discontinuation after treatment,the TCM syndrome integral of the treatment group was lower than control group and the difference was statistically significant(P<0.05).However,the TCM syndrome integral of the control group returned to the state before the treatment within three months of drug withdrawl(P>0.05).③The improvement of menstrual cycles: the menstrual cycle score in the two groups after the treatment and three months’ drug withdrawl decreased compared with before treatment, and the score in treatment group was lower than in control group,and the difference was statistically significant(P<0.05).④Sex hormone levels: the levels of T,LH,LH/FSH,DHEAS were obviously lower after treatment than before in the two groups,and the difference was statisticallysignificant(P<0.05).However there was no statisti- cal difference in E2 and FSH between the two groups(P>0.05).⑤Indexes of glucose metabolism: there were no statistical difference between the two groups in FPG,FINS,HOMA-IR levels(P>0.05).⑥Hairy、acne situation: the score of hirsutism and acne slightly decreased after treatment than before treatment in the two groups,however there were no statistical difference between the two groups(P > 0.05). ⑦Polycystic ovary condition:to change the volume ovarian volume and two ovarian antral follicle number two groups of patients before and after treatment, the difference was statistically significant(P < 0.05). ⑧ Safety evaluation:there were 2 patients occurred mildly nausea and vomiting in control group,the symptoms alleviated spontaneously without any special treatment.30 patients in the treatment group did not appear adverse reaction. After treatment, liver and kidney function, the samples of blood, urine and stool and electrocardiogram showed no obvious abnormalities.Conclusion: Using compound Xuan Ju capsule combined Diane-35 in the treatment of PCOS with kidney yang deficiency can significantly improve the TCM symptoms of kidney Yang deficiency, lower levels of serum T,LH,DHEAS,LH/FSH,reduce ovarian volume and numbers of two ovarian antral follicles, make menstrual cycle regular and has less side effects as well as durable’s curative effects.It has been proved that combination of traditional Chinese and western medicine in the treatment of PCOS has good clincial curative effect.
