Objective: Liver cirrhosis caused by hepatitis B is a chronic liver disease caused by hepatitis B virus infection leading to liver cell degeneration necrosis and liver tissue diffuse fibrosis, having pathological features of tubercles and pseudolobuli. Ascites is the most prominent clinical manifestation of liver cirrhosis, caused by porto-hyperpressure and over 75% of the patients in cirrhosis decompensation stage have ascites. Refractory ascites is a common complication of end-stage chronic liver disease, not only it seriously affects the patient’s quality of life, but it also can lead to infection and upper gastrointestinal bleeding, hepatic coma, renal failure and other serious complications with high fatality. Now,there is no effective way in modern medicine to control disease progression and recurrence.Guided by the theory of Traditional Chinese Medicine(TCM) this study applied mainly Xie-shi liver-softening & water-retention alleviating prescription combined with modern medicine treatment to liver cirrhosis with refractory ascites caused by hepatitis B virus, to observe and evaluate clinical effects and explore the possible mechanism, to provide some theoretical and practical basis for the treatment of the disease.Methods: All cases of this study were inpatients of Dalian No.6 people’s Hospital from January 2013-Septenber 2014, had liver cirrhosis with refractory ascites, according to the diagnostic criteria, inclusive criteria and exclusive criteria, 65 patients were selected by signing the informed consent form, they were randomly divided into control group(30 cases)and treatment group(35 cases).There were no statistical differences in clinical parameters in two groups. The control group accepted anti-viral, liver protective, diuresis, reducing symptomatic and supportive treatment, the treated group accepted antiviral and general supportive therapy plus Xie-shi liver-softening & water-retention alleviating prescription treatment, both groups had been treated for a month and had one month follow-up after that.We observe two groups before and after treatment, and compare total efficiency, clinical symptoms improvement, recurrence rate and liver function index(Alt, serum total bilirubin, albumin) and ascites quantity(all patients took ultrasound scan in horizontal position).The comparison between the two groups was conducted by t test and x2 test. Explore that if Xie-shi liver-softening & water-retention alleviating prescription treatment combining modern medicine treatment is better than modern medicine treatment in treating liver cirrhosis with refractory ascites caused by hepatitis B virus.Results: 1 All 65 patients completed their treatment and follow-up courses. 2 The treated group compared with the control group, the significant efficiency rate was 68.3% vs 35.5%(P < 0.05),the effective rate was 85.2%vs 85.2%(P < 0.05), the clinical symptoms improvement rate was 87.2% vs 54.1%(P < 0.05), the recurrence rate was 12.5% vs 33.4%(P <0.05).The above indexes showed that the effect of treated group is better than that of the control group, the difference was statistically significant. 3 The patients’ liver function index of the treated group shows that jaundice subsided and albumin increase are better than that of the control group, the difference is statistically significant(P < 0.05).There is no statistical significance in ALT measurement between two groups(P >0.05). 4 Ascites reduction index: ascites subsided effect of the treated group is better than that of control group, the difference is statistically significant(P < 0.05). 5 Cyto-immune index:IL-2 and interferon of treated group is better than that of control group, the difference is statistically significant(P < 0.05).Conclusion: Xie-shi liver-softening & water-retention alleviating prescription treatment has better effect than modern medicine treatment for liver cirrhosis with refractory ascites caused by hepatitis B virus. Xie-shi liver-softening & water-retention alleviating prescription has no adverse reactions, and obviously improves the patient’s clinical symptoms and liver function, shortens hospitalization time, reduces the cost, decrease relapse, has short term and long term effect, and provides better methods for liver cirrhosis with refractory ascites caused by hepatitis B virus.
