With the trend of population aging in our country becomes more and more intense, the traditional pension model has been unable to meet the older people’s growing demand for pension services. Therefore, our country began to explore the new pension pattern suitable to the situation of our country, which on the family pension mode become typical representative. The social pension system emphasizes the main body position of the home-based care, and emphasizes the responsibility of all parties in the society.Objective:This article is about the rural elderly health status and service demand investigation, and care needs of elderly people in Wuxi City survey, from living needs, demand for medical care, nursing demand service among older people in Jiangsu Province to learn the current pension demand conditions. According to the service needs of the elderly, the specific content of the social pension system is integrated and expanded. From the perspective of welfare pluralism theory, combined with the empirical analysis of the data, from the three aspects of the social security network, home care, the social pension service force, a comprehensive analysis of the social pension system of the basic framework, and put forward suggestions for relevant policies.Subjects and methods:An Empirical Analysis of the first part for the rural elderly health status and service demand survey:survey data source in the "World Bank loan/UK grants China rural health development project", the survey content relates to the basic situation, health services and, lifestyle, two week prevalence and two weeks treatment, health self evaluation. The second part in elderly people in Wuxi City Survey of nursing needs:a sampling survey was conducted in Wuxi City 7 elderly population. The main contents of the survey include the basic situation, health, care facilities, cost of care.Results:1. Survey of health status and service needs of the elderly in rural areas:(1) Two weeks prevalence rate is 48.9%, of which 72.7% were chronic diseases (hypertension or diabetes). (2) The results of the European five dimensional Health Inventory (EQ-5D) showed that five dimensions of each, more than 80% of the elderly were selected without difficulty,2 792 elderly residents of the VAS average score of 74.9 points. (3) Self rating health score and the influencing factors of the multiple regression analysis showed that:suffering from chronic disease, old age (over 80 years old), action barrier, engaged in daily activities obstacles, feeling pain or discomfort and anxiety the elderly self rated health scores were lower. The scores of drinking, education and work of the elderly were higher than those of the self rated health. (4) Smoking in elderly residents accounted for 28%, the weekly exercise of elderly residents accounted for relatively low, only 16.7%.2, Wuxi elderly care needs survey:(1) 5.1% of 12 581 elderly residents need nursing.80 of the 20.3% year old and older people can not take care of themselves, that there is a need for care of the elderly in each of the five. (2) Most of the elderly people need to care for chronic diseases, or chronic diseases caused by partial or total loss of energy. (3) The elderly per capita cost of 21882 yuan per capita,6 out of the elderly per capita care costs for 36382 yuan. (4) Most of the elderly can not take care of themselves at home, only 11.3% of the choice of living pension institutions.49.9% choice in home care,29.7% of the elderly to choose in the hospital (including rehabilitation hospital) nursing.Recommendations:(1) Chronic disease patients, elderly (over 80 years of age), the action has an obstacle, engage in daily activities, physical pain or discomfort, feeling anxious elderly is the focus of service provision. (2) To strengthen the education of healthy lifestyle, to promote the re employment of the elderly, to enrich the lives of the elderly. (3) Nursing services should be more targeted and professional, for the elderly and patients with chronic diseases should be developed characteristics of nursing service menu, there is a fixed home care staff to provide the appropriate care services. (4) The higher the disability level, the higher the cost of nursing, the burden of the cost of care. To actively explore the establishment of long-term care insurance system, this part included in the scope of reimbursement, to further reduce the burden of disease in the elderly. (5) To strengthen the popularization of family care knowledge, and medical institutions to complement each other.
