This paper takes old people from all subordinate communities of B Street in Suzhou city as the object of study. The elderly in community mostly belongs to empty nesters—husband and wife live together or one person lives alone and their children have no time to visit often. With the decline of body functions of old people, some chronic diseases affect their life quality. They lack the care and the need to be loved and their physical health is also under threat, which shows that health service is inadequate. Thus with the intervention of Community-care Model and group work, this paper aims to alleviate the phenomenon of health services lackage for the elderly in the community through the carry out of love&care of "H health care action" and solicit good advice lecture hall and other project activities.In the process of Social Working Method intervention, workers evaluate the effect of each project activity and find the problems like insufficient professional skills of the social workers in the project operation and the participation of the service objects and others through questionnaires and interviews timely so that they can take corresponding measures to deal with all these problems. Through the analysis and evaluation of the data comparison, the attitude of service object as well as the aggregation of team members before and after the project, it proves that in carrying out "H health care action", it played an instrumental role in health service for the elderly in the community that can effectively meet the health needs of them. At the same time, through the development of the project, it analyzed the benefits and drawbacks of "H health care action" and the problems of the intervention of Social Working Method and found the corresponding countermeasures to integrate various resources to intervene the problems for the aged in the community.
