Study On The Relationship Between 102 Patients With Idiopathic Asthenozoospermia And Teratozoospermia And TCM Constitution Identification

ObjectivesTo explore the distribution rule of TCM constitution identification in patients with idiopathic asthenozoospermia and teratozoospermia, and to trace observation on their clinical effect observation in order to analy and verificate their correlations.MethodsWe study of 102 cases, meeting the inclusion criteria, are the patients aged between 25 and 48 from the urology surgery, andrology clinic and reproductive medicine clinic in Guangdong Provincial Hospital of TCM from September 2015 to April 2016. After taking back the clinical obsercing table, which include basic information and TCM constitution scale, and recording their semen analytic result, we analysis the statistical relation among the basic information, TCM constitution and semen analytic result by using statistical methods. In addition, by following up some patients'curative effects, we validate the analytical results.Results1. There are 352 TCM constitution factors among these 102 patients. The number of single TCM constitution type is 23 (23%), while the number of mixed TCM constitution type is 79(77%).The proportion of each physical is damp-heat constitution(17%), qi-deficiency constitution(16%), qi-stagnation constitution(12%), yin-deficiency constitution(11%), phlegm-dampness consti tution(ll%), gentleness constitution(10%), blood-stasis constitution(9%), yang-deficiency constitution(8%), special-innatenessconstitution(6%).2. Among these 102 patients,there are 37 patients(36%) with symptom and 65 patients(64%) without symptom.3. The common TCM constitution in symptomatic patients, who have a total of 125 TCM constitution factors, are damp-heat constitution(20%) and qi-deficiency constitution(20%). In addition, there are 227 TCM constitution factors among asymptomatic patients, and the common TCM constitution in them are damp-heat constitution(15%) and qi-stagnation constitution(15%).4.Among the symptomatic patients, the proportion of patients with qi-deficiency constitution is higher than those without qi-deficiency constitution(45% VS 26%),,P<0.05. And among the asymptomatic patients, the proportion of patients with qi-stagnation constitution is higher than those without qi-stagnation constitution(75% VS 55%), P<0.05. There is no statistical signigicance between symptom and damp-heat constitution, yin-deficiency constitution, phlegm-dampness constitution(P>0.05).5. By the ran-sum test or student's t-test, there is statistical signigicance between qi-deficiency constitution and sperm viability, total sperm vital ity(P<0.05), as well as between damp-heat constitution and sperm concentration(P<0.05). However, there is no statistical signigicance between the main TCM constitution and forward moving sperm, the rate of nomal morphological sperm(P>0.05).6. By the pearson correlation analysis or spearman rank correlation analysis, there is a significant positive correlation between sperm viability and total sperm vitality, forward moving sperm(P<0.01), as well as between sperm concentration and the rate of nomal morphological sperm(P<0.01). On the contrary, sperm concentration and total sperm vitality, sperm viability have no statistical significance(P>0.05), nor do sperm vitality and the rate of nomal morphological sperm(P>0.05).7. The part of observing curative effect includes in a total of 50 cases, including 15 symptomatic patients and 35 asymptomatic patients. After analysing the distribution of their main therapies, there are 14 cases with reinforcing qi therapy,13 cases with promoting qi therapy,13 cases with clearing heat and draining dampness therapy,8 cases with nourishing Yin therapy and 3 cases with eliminating phlegm and remove dampness therapy. After treatment, there are 29 patients having curative effect, while other 21 patients have no significant curative effect.8. By the way of the ran-sum test between main therapies and curative effect, we draw a conclusion that among these patients, using clearing heat and draining dampness therapy, reinforcing Qi therapy and promoting qi therapy has statistical significance with curative effect (P?0.05). Furthermore, there is a significant positive correlation between using promoting qi therapy and curative effect among asymptomatic patients (P<0.05), but there is no statistical signigicance between using reinforcing Qi therapy and curative effect among asymptomatic patients (P?0.05).Conclusion:Among the patients suffering from idiopathic asthenozoospermia and teratozoospermia, the most common TCM constitution are damp-heat constitution, qi-deficiency constitution and qi-stagnation constitution. There may be a certain relationship between the asymptomatic patients and qi-stagnation constitution. Consequently, during the treatment for these patients with idiopathic asthenozoospermia and teratozoospermia, we should pay attention to the using of clearing heat and draining dampness therapy, reinforcing qi therapy and promoting qi therapy. Further more, we should pay more attention to use reinforcing qi therapy in asymptomatic patients.
