Background and PurposeProstate hyperplasia as the most common male urological diseases,including urinary frequency,urgency,dysuria,urinary incontinence and other symptoms of urinary obstruction and urinary retention even hydronephrosis,renal insufficiency and other problems caused by prostate hyperplasia have plagued many people.At present,the treatment of benign prostatic hyperplasia has made great progress,but its mechanism research is still lagging behind.Previously thought that age and androgen are the most critical factors affecting the disease,and the emergence of drugs that blocking androgen improve the symptoms of urinary obstruction.But a large number of case data and preliminary studies confirmed that benign prostatic hyperplasia is the result of many factors together,inwhich chronic prostatitis as the basis of benign prostatic hyperplasia has been supported by many research results.The aims of this study were to explore the role of high-mobility group box 1(HMGB1)in benign prostatic hyperplasia(BPH)and to perform a preliminary investigation of the mechanisms underlying BPH.MethodPatients chosen from the first affiliated hospital of Zhengzhou University,urology in March 2014 to June 2015 according to strict inclusion criteria,which met the requirements of the subjects were 160 cases.Based on a certain inclusion criteria,random screening of eligible patients with benign prostatic hyperplasia in 160 cases.Which included the standard is mainly combined with the patient symptoms,signs,imaging performance,urodynamic functional test results support.Discharge criteria mainly for a history of prostate surgery,lower urinary tract invasive operation history and a strong suspicion of prostate cancer.The volume,PSA level and I-PSS score index of all patients were measured,statistically analyzed and analyzed in detail.Prostate volume to take the abdomen and rectal color Doppler ultrasound-based,magnetic resonance(MRI)supplemented by the standard.PSA level testing through blood tests,pay attention to grasp the timing of PSA testing,otherwise the results should be considered invalid,if necessary,to be repeated.All patients undergoing this study underwent transurethral resection of the prostate(TURP)and postoperative pathologic examination.Immunohistochemistry and Western blotting were used to detect the expression of HMGB1 in prostate tissue of 160 patients.After we cultured in vitro prostate epithelial cells to obtain 0,3,6,12,24 and 48 hours of tissue cells and cell culture medium.The expression levels of HMGB1 in prostate epithelial cells and surrounding culture were detected by protein and enzyme immunoassay(Elisa)respectively at 0,3,6,12,24 and 48 hours,respectively.At the same time,we used carboxyfluorescein succinimidyl ester(CFSE)method to detect the proliferation of cells.Finally,we used t-test,one-way or two-way to test the data differences between the different groups.Spearman correlation analysis was used to assess the relationship between HMGB1 protein expression and related clinical factors in prostate tissue.ResultsIn the 160 patients who underwent postoperative pathology,we found that 86 out of 160 patients diagnosed with BPH with chronic prostatitis and 74 were diagnosed with simple BPH.The expression level of HMGB1 in benign prostatic hyperplasia patients with prostatitis was higher than that in patients with benign prostatic hyperplasia(P <0.05).Spearman correlation analysis showed that the expression level of HMGB1 was positively correlated with the size of prostate(P <0.05),and there was no significant correlation with age and I-PPS score in prostatic hyperplasia patients with prostatitis The There was no significant correlation between HMGB1 expression level and prostate volume,age and I-PPS score in simple benign prostatic hyperplasia group.The results of protein labeling and enzyme-linked immunosorbent assay(ELISA)showed that the expression of HMGB1 in prostatic epithelial cells and the effect of release into the surrounding culture medium were significantly enhanced after the action of lipopolysaccharide(LPS)Hour reached its peak,then descended.In addition,overexpression of HMGB1 promotes the proliferation of prostate epithelial cells under LPS stimulation,which can be inhibited by HMGB1 antagonist boxA.ConclusionHMGB1 may be associated with benign prostatic hyperplasia,and inflammation may induce the expression of HMGB1 and release it from the cell.The expression of HMGB1 may be related to the proliferation of prostatic hyperplasia,and the expression of HMGB1 may be related to the proliferation of prostatic hyperplasia.HMGB1 plays an important role in benign prostatic hyperplasia and chronic prostatitis,which provides a new idea for the pathogenesis of benign prostatic hyperplasia,and also provides a new idea for the prophylaxis of benign prostatic hyperplasia,which also leads to the proliferation of prostate epithelial cells,leading to benign prostatic hyperplasia.Indicating that HMGB1 can serve as a biomarker of potential prostate hyperplasia and therapeutic targets.
