Following the continuous aggravation of aging population degree in China, the traditional family supporting mode is facing huge challenges. The existing institutions for old-age care also can not satisfy the care needs. In comparison, home-based care for the aged is much more conforms to the national circumstances. Home-based care for the aged takes family as the core, the community as support, the professional service organizations as carriers to provide socialized services majoring in daily care, rehabilitation nursing, medical care to the elder people at home in the form of visiting, day care or neighborhood help. In order to better study the home-based care services problems in T community, this research made a questionnaire to elder people of 60 years old or above, and made a empirical research on the T community home-based care services problems.This paper studied urban community home-based care for the old in three aspects. Firstly, it stated the research background and significance of urban community home-based care for the aged and summarized research achievements and practical experiences. Secondly, according to field investigation data and interview materials in T community, it analyzed the service demands and community supply of home-based care for the aged in daily care, medical care and spiritual consolation, and the problems and reasons exiting in implementing community home-based care for the aged. Finally, according to the T community home-based care services status and problems put forward suggestions and Countermeasures to improve the city community home care services.To promote the urban community home care services to provide useful practice guidance.
