Research On The Model Of Urban Community Home Care Combined With Old-age Care

In 2000,China stepped into the aging society,and presents Chinese characteristics of huge elderly population,aging trend and increasing disabled elderly.In order to deal with the problem of aging population,China has established a social pension services system,which is based on home,supported by community,and supplemented by organization.Because the physiological function of the elderly gradually decline,they are vulnerable to disease,and urgent to combine the medical services with the pension services.the current medical services and pension services for the elderly in China are separated.In the face of "old-age pension institutions do not provide health care","medical institutions do not provide pension services" and the elderly have strong demand for community endowment,China should vigorously develop community home endowment combined with medical services.Based on healthy aging theory,demand theory,welfare pluralism theory,and embedded theory,summarizing the results of previous studies,this paper takes Xi'an city as an example,and study on city community home pension services combined with medical services.First,we analyze the present supply situation of Xi'an community home pension services combined with medical services,and find that there are some problems,such as inadequate supply of medical and care resources,vague service standards and so on.secondly,through questionnaires and interviews obtain first hand data of the community home pension services combined with medical services of Xi'an.From four angles--the basic personal information,economic status,health status,cognitive concepts,we analyze the basic situation of the elderly of Xi'an,and further analyze the demand willingness and content of the community elderly people for pension services combined with medical services.From data we can see the elderly urgently need such combination.Thirdly,according to the analysis of the supply and demand status of community home pension services combined with medical services in Xi'an,we put forward the basic framework and implementation paths of such combination.The basic framework includes taking the community as the carrier,integrating medical and care resources in the community,and establishing the service platform.The implementation paths includes the cooperation of community health institutions and pension service center,responsibility system of family doctors,mini nursing homes and so on.finally,in order to ensure the smooth implementation of the community home pension services combined with medical services,we put forward some corresponding suggestions,such as the service policy construction,services supply,the participation of social forces and the construction of talent team.
