Case Study Mode Of Action Research On Rehabilitation Of Elderly Stroke Patients

Disease cured,effective communication with the medical staff,treatment,and adverse reactions of coping,physical rehabilitation,diet and exercise,fear of illness aggravating,psychological support,the problem such as marriage,family links throughout the process of rehabilitation in elderly patients with cerebral apoplexy,and continue to affect the life of patients.Therefore,to explore a new working mode,for diseases in elderly patients with cerebral apoplexy of individualized characteristic and the demand,satisfy their support for health education,information and psychological counseling,rehabilitation guidance long-term demand,realize the continuous,individualized,dominated by the patient's overall service,has become a new challenge in the field of stroke rehabilitation.The aim of this study is through the use of ecological system theory,theory of social support,etc.,based on the needs of elderly and adopts the model of case management of social work,the actual demand for understanding the rehabilitation in elderly patients with cerebral apoplexy.From the perspective of action research,to construct the service mode of medical social workers,perfect is adapted to social economic development level,and can meet the demand of the elderly character of case management system,has important theoretical and practical significance.This thesis mainly is hope that through some specific case management model in elderly patients with stroke rehabilitation practice to carry out,use of its functions of professional,at the same time for practice of social work in the elderly diseases caused some problems in rehabilitation for reflection and thinking.Research has shown that case management model in the rehabilitation process of patients with senile cerebral apoplexy,in disease rehabilitation effect,personal emotion management,the relationship between family members,reconstruction of social resources,etc,all have more obvious effect.
