Objective:To investigate the clinical and molecular epidemiological characteristics of rotavirus in children acute diarrhea in Chongqing in2016,and monitor dynamically the infection of rotavirus among children in Chongqing,providing a basis for the recognition,prevention and treatment of rotavirus infection and the selection of the best vaccine in Chongqing.Methods:From January to December in 2016,a total of 505 fecel samples were collected from children with acute diarrhea conforming to the inclusion criteria at the outpatient department of the Affiliated Children's Hospital of Medical University Of Chongqing,the relevant clinical data of the corresponding children was obtained simultaneously.All samples were detected by ELISA for group A rotavirus,and then the rotavirus-positive specimens were tested by RT-PCR to classify the genetypes of VP7(G)?VP4(P).Results:1.A total of 505 fecal samples(male 327,female 178)were collected,and 162 fecal samples including 97 male and 65 female were discovered to rotavirus-positive by ELISA,the positive rate of rotavirus in 2016 was 32.08%(162/505),and the positive rate of rotavirus in male and female respectively were 29.66% and 36.52%,there was no significant difference in the gender distribution(?2=2.485,P=0.115).In 162 cases,infants and children under 2 years old accounts for 92.59%,and 0~6?6~12?12~24?24~36?36~48?48~60?over 60 months of age successively accounts for 19.75%(32/162)?38.89%(63/162)?33.95%(55/162)?2.47%(4/162)?3.09%(5/162)?0.62%(1/162)?1.23%(2/162),the positive rate of RV in each age group respectively is 32%(32/100),30.29%(63/208),39.57%(55/139),17.39%(4/23),31.25%(5/16),10%(1/10),22.22%(2/9).RV enteritis occurred in all months of the year,and The positive rate of RV was the highest in winter,espec ially in December and January.2.The counts of RV enteritis children in 162 cases with vomiting,fever,respiratory symptoms were 83 cases,56 cases,62 cases,the pos itive rates were 51.23%(83/162)?38.27%(62/162)?34.57%(56/162),res pectively.64.2% of the children had a doctor'office visiiting within 3days of the diarrhea course,and 82.72% of them saw a doctor within 5 days of the diarrhea course.It's most common that children suffered from diarrhea 4~6 times a day,accounting for 46.91% of the total cases of RV diarrhea,and 10 times a day accouting for 94.44%.In most cases,the number of days of vomiting was 1~2 days(81.93%,68/83).The number of cases within 6 times of daily vomiting accounted for 90.37%.The degree of fever accompanying RV enteritis was do minated by middle and low heat(80.35%,45/56).3.In 162 fecal samples identified rotavirus-positive by ELISA,there were 71 cases which succeeded in gene amplification and sequencing,and were found that there were five types of G genotypes?two types of P genotypes and five types of G/P genotypes combination,acco rding to VP7(G)and VP4(P)genotypes classification.In G genotype s,there was G9(77.46%,55/71)?G3(7.04%,5/71)?G1(2.82%,2/71)?G2(2.82%,2/71)?G4(1.41%,1/71),ungenetype accounted for 8.45%(6/71).In P genotypes,there was P[8](83.10%,59/71)?P[4](1.41%,1/71),ungenet ype accounted for 15.49%(11/71).In G/P genotypes,there was G9P[8](67.61%,48/71)?G3P[8](4.22%,3/71)?G1P[8](1.41%,1/71)?G2P[8](1.41%,1/71)?G2P[4](1.41%,1/71),ungenetype accounted for 23.94%(17/71).All genotypes were detected mainly in the autumn and winter seas on,but were found at least in summer.Conclusion:The incidence of RV enteritis among children in Chongqing was still high in 2016,Infants and young children under two years of age were the main target of RV infection.The occurrence of RV enteritis is obviously seasonal,the peak of onset is in winter.According to the results of statistical analysis,it's found that Vomiting is closely related to RV enteritis,but fever and respiratory symptoms have no significant correlation with RV enteritis.In G genotypes,G9 was the dominantepidemic strain,followed by G3,G1,G2,G4.In P genotypes,P[8] was the dominant epidemic strain,followed by P[4].In the G/P genotype combination,G9P[8] was the dominant epidemic strain,followed by G3P[8],G1P[8],G2P[8] and G2P[4].The G/P genotyping of RV in different years is diverse,but the dominant strain of all the genotypes in the 3 years from 2014 to 2015 were the same(G9,P[8],G9P[8]),the P genotype of the three years was relatively stable.
