Relationship Between Serum SST2,TGF-β1,Galectin-3 And Left Ventricular Hypertrophy In Elderly Patients With Hypertension | | Posted on:2020-01-06 | Degree:Master | Type:Thesis | | Country:China | Candidate:Z H Wang | Full Text:PDF | | GTID:2404330596997109 | Subject:Internal medicine | | Abstract/Summary: | PDF Full Text Request | | Objective This study was designed to analyze the relationship between serum soluble ST2(sST2),transforming growth factor beta 1(TGF-β1),galectin-3(Galectin-3)and left ventricular hypertrophy in elderly hypertensive patients,and explore their value in the diagnosis of elderly hypertension complicated with left ventricular hypertrophy.Methods A total of 160 elderly patients diagnosed with hypertension who were diagnosed with hypertension in the STEP group participated in the study.Left ventricular end-diastolic diameter(LVEDD),end-diastolic left ventricular posterior wall thickness(LVPWT),end-diastolic interventricular septal thickness(IVST),and left ventricular mass index(LVMI)were measured by echocardiography.According to the Expert Consensus on the Diagnosis and Treatment of Hypertension and Left Ventricular Hypertrophy in Asia,they were divided into three Groups: A,B,and C.The Group A was Hypertension with left ventricular hypertrophy: LVMI≥115 g/m2(male)),LVMI≥95 g/m2(female);Group B was early hypertension combined with left ventricular hypertrophy: IVST or LVPWT [≥11 mm(male),≥10 mm(female)] and has not reached the diagnostic criteria for hypertension and left ventricular hypertrophy;Group C was hypertension non-left ventricular hypertrophy group:those who did not meet the diagnostic criteria of group A and B.All subjects underwent echocardiography and calculated left ventricular mass index(LVMI).All subjects were tested for biochemical indicators such as liver and kidney function,blood sugar and blood lipids on an empty stomach.Serum sST2,TGF-β1 and Galectin-3 levels were determined by ELISA.(1)Compare the clinical baseline levels of patients in groups A,B,and C.(2)Comparison of serum sST2,TGF-β 1 and Galectin-3 levels in patients with the three groups.(3)To analyze the diagnostic value of serum sST2,TGF-β1 and Galectin-3 for left ventricular hypertrophy.(4)The correlation between LVMI and sST2,TGF-β1 and Galectin-3 was analyzed.Results(1)The IVSTD,LVPWTD,LAD,LVDS and LVDD in group A were higher than those in group B and C.The difference was statistically significant(p<0.05).The level of IVSTD in group B was higher than that in group C(p<0.05).There was no significant difference in LVEF and FS levels between the three groups(all P>0.05).(2)The levels of sST2,TGF-β1 and Galectin-3 in group A were higher than those in group B and C,and the differences were statistically significant(all p<0.05).(3)The area under the ROC curve of sST2 diagnosis of left ventricular hypertrophy was0.727(95% CI: 0.626-0.829,p<0.01),and the diagnostic value was the highest.The area under the ROC curve for the diagnosis of left ventricular hypertrophy by TGF-β1 was 0.718(95% CI: 0.627-0.810,p<0.01);The area under the ROC curve for the diagnosis of left ventricular hypertrophy by Galectin-3 was 0.721(95% CI: 0.612-0.829),p<0.01).(4)According to Spearman correlation analysis,LVMI was associated with TGF-β1(r=0.159,p=0.044);low correlation with Galectin-3(r=0.242,p<0.05);moderately correlated with sST2(r=0.334,p<0.05).Multivariate linear stepwise regression analysis showed that sST2 was independently and positively correlated with LVMI.Conclusions(1)Serum sST2 may be a biomarker for diagnosing elderly hypertension with left ventricular hypertrophy and assessing the severity of left ventricular hypertrophy.(2)Compared with TGF-β1 and Galectin-3,serum sST2 has a better diagnostic value for hypertension combined with left ventricular hypertrophy. | | Keywords/Search Tags: | Left ventricular hypertrophy, Elderly hypertension, Soluble ST2, Transforming growth factor β1, Galectin-3 | PDF Full Text Request | Related items |
