Clinical Study On The Effect Of Antibacterial Photodynamic Therapy On The Treatment Of Moderate To Severe Periodontitis And IL-18 In Gingival Crevicular Fluid

Objective: The purpose of this study was to observe and compare the clinical efficacy,the amount of gingival crevicular fluid and the concentration of interleukin-18 in the treatment of moderate and severe periodontitis between the two groups of Antimicrobial photodynamic therapy(a PDT)with periodontal basal therapy and periodontal basal therapy only,so as to explore a more favorable new Antimicrobial pathway for the treatment of periodontal disease.Methods: A total of 22 patients(9 males and 13 females)who were diagnosed with chronic periodontitis and were admitted to the periodontal department of stomatological hospital affiliated to nanchang university on July 1,2019 to December 31,2019 were selected,The average age is 38.73 Y.Four teeth with similar lesion conditions were selected for each patient,and the affected teeth were preferentially selected as the homonymous posterior teeth in each quadrant.The affected teeth on the left and right sides were randomly divided into the experimental group and the control group for their own control,with two affected teeth in each group.The treatment measures of the experimental group were a PDT+ periodontal basic treatment,and one photodynamic therapy was performed 1 and 2 weeks after the basic treatment,while the control group only received periodontal basic treatment,and a PDT was irradiated with photodynamic instrument at the same time.Periodontal clinical indicators : probing depth,probing bleeding index and biochemical indicators: mass of gingival creval fluid and interleukin-18 concentration at baseline and 4 weeks after a PDT were examined and recorded,and changes in appearance and texture of gingival tissue were observed.Results: One month after treatment,the periodontal status of all patients was improved significantly,and the clinical and biochemical indexes were also significantly reduced.There was no significant difference in PD and BI between the two groups at baseline.At baseline,PD in the experimental group decreased from 5.98±0.93 mm to 4.41±0.82 mm,and the difference in PD value between the experimental group and the control group at 4 weeks after treatment was statistically significant(P < 0.05),BI in a PDT+SRP group decreased from 3.56 ± 0.50 to 1.81 ± 0.79,and there was a statistical difference between the two groups 4 weeks after treatment(P < 0.05).At baseline,the amount of gingival crevicular fluid in the antibacterial photodynamic therapy group was 2.16 ± 0.53 mg,and the concentration of Il-18 in gingival crevicular fluid was 305.16±79.64pg/ml,which decreased to 0.76±0.23 mg and 97.74 ± 26.56 pg/ml at 4 weeks after treatment.The differences between the groups before and after treatment and 4 weeks after treatment were statistically significant(P < 0.05).Conclusion:After the auxiliary use of a PDT in periodontal basic therapy,the clinical indicators of BI and PD in patients showed a significant decline.a PDT as an auxiliary measure of periodontal basic therapy can improve the periodontal condition.After 1 month of adjunct a PDT treatment,the amount of GCF and the concentration of IL-18 in GCF decreased significantly,and the decrease was more significant than that in the control group,which may help to control periodontal inflammation in the short term.
