Abstract/Summary:
BackgroundAs an important teaching and training link,short-term maritime drill course is a necessary subject formedical university students to become qualified graduates.Short-term maritime training is also an important form of current maritime tasks carrying out,which includes early preparation,maritime training and recuperation after training.Maritime course drills,as a stressor,can have varying degrees of impact on the health of trainees,especially lifestyles such as sleep.It is of great significance to solve the influence of short-term offshore curriculum drill on students’ health,especially the influence of the whole process of offshore curriculum drill on students’ sleep quality,to effectively carry out offshore curriculum drill,enhance the training effect,and maintain students’ physical and mental health.Objectives:1.To analyze the current situation of sleep quality in two periods(campus gathering period and sea drilling period)of marine curriculum drill for students of a medical university,and the factors of influencing sleep quality in those two periods;2.Compare and analyze the PSQI score of sleep quality and the differences of seven factors in sleep quality between campus gathering period and sea drilling period;explore the similarities and differences of sleep quality between campus gathering period and sea drilling period and their causes;3.Propose countermeasures and suggestions for improving sleep quality in different periods of marine curriculum drill for students,providing a basis and direction for the intervention measures of sleep quality during marine curriculum drill.Methods:Adopt the general condition questionnaire,Pittsburgh Sleep Quality Index,Self-Rating Anxiety Scale,Self-Rating Depression Scale,Cognitive Emotion Regulation Questionnaire to investigate the campus gathering period and offshore drill period of a medical university.Convenient sampling method was used to select the graduates of a medical university who participated in the sea course drill,and a questionnaire survey was conducted on the day of campus gathering and the end of the sea drill period.The results of the survey were analyzed by using SPSS 21 statistical software.Measurement data meeting normal distribution are expressed by x ?????± s,and enumeration data of non-normal distribution are expressed by percentage.Independent sample t test was used to compare measurement data that met normal distribution;Wilcoxon test was used to compare measurement data that did not meet normal distribution,and χ2 test or contingency table was used to analyze enumeration data.Spearman bivariate correlation was used to analyze the correlation between the scores of cognitive emotion regulation strategyand the PSQI total score at different periods.Multivariate logistic regression(backward: Wald method,inclusion criteria α = 0.05,exclusion criteria α =0.10)and ordinal regression were used to analyze the effect of each influencing factor on students’ overall sleep and sleep factor scores,and the odds ratio(OR)of each risk factor was calculated.Cronbach’s Alpha reliability coefficient and split-half reliability coefficient were used for reliability analysis of anxiety,depression,and cognitive emotion regulation scales,and deterministic factor analysis was used for construct validity analysis of cognitive emotion regulation scales.P < 0.05 was considered statistically significant.A purposive sampling method was used to select nine managers and 21 students who complained of severe sleep disorders during the maritime drill period for in-depth interviews.The proposed interview outline "What are the main symptoms of sleep disorders of school students during the maritime course drill?" "What do you think are the factors that affect students’ sleep quality? " "What measures do you think can improve students’ sleep quality? " The three questions were administered semi-open-ended interviews and meaningful results were extracted.Results :1.The PSQI score(6.60 ± 2.43 points)and the detection rate of sleep disorders(PSQI > 7,29.00%)were higher than those of their peers during the campus preparation period,the sleep quality was poor during the short-term drill action preparation period.The risk factors of sleep quality during the campus preparation period of students were learning stress,physical illness,anxiety,self-blame,and blame others,and the protective factors were rational analysis.2.The PSQI score(6.64 ± 3.09 points)and the detection rate of sleep disorders(PSQI > 7,33.73%)of students during offshore drill period were lower than those of crisis stress events and long voyage personnel,the effect of short-term drill action on sleep quality was lower than that of long voyage.The risk factors for sleep quality during students’ maritime drills were seasickness,bedbound experience,irregular personal routines,depression,self-blame,and contemplation,and the protective factor was acceptance.3.There was no significant difference in sleep quality scores between the two periods(χ2 = 2.318,P = 0.128).There were significant differences in the scores of subjective sleep quality,sleep duration,sleep efficiency,and sleep disturbance between the campus preparation period and the offshore drill period(P < 0.05).4.Interview results: The sleep disorders of medical students participating in offshore drill courses are mainly manifested in sleep initiation difficulties,sleep maintenance difficulties,early awakening,and daytime dysfunction.Sleep quality is mainly affected by environmental factors,psychosocial factors,irregular personal life routines,bed recognition,seasickness,genetics and other factors.5.Sleep quality management countermeasures and suggestions: Students’ sleep quality can be improved through usual sleep course lectures,visiting and seeing,article preparation preaching,psychological preparation,psychological counseling,sleep self-management,and accompanying sleep aids.Conclusion:1.The problem of sleep quality of medical students in maritime curriculum drill is common.Organizers should attach great importance to the sleep quality of students during the maritime drill course.2.There was no difference in the overall sleep quality between the student campus gathering period and the maritime drill period,but there were differences in subjective sleep quality,sleep duration,sleep efficiency,and sleep disorder scores among the sleep factors.3.Popular science education on healthy sleep and reasonable implementation of political thoughts and mental health should be done well in the whole process of peacetime and maritime curriculum drills.The medical student campus gathering period can improve sleep quality by reasonably arranging learning tasks,relieving learning pressure,reducing the incidence of physical diseases,and preparing for departure.Medical students can take preventive measures to reduce the discomfort caused by seasickness,reducing bed acclimatization,improving daily routines,reasonable diet,reasonable allocation of sleeping aids and sleeping pills among the team.
