With the trend of aging and family miniaturization,there are more and more empty nesters and the elderly living alone,and the problem of supporting and rehabilitation of many diabetic elderly has become the main problem of community care.Type 2 diabetes,which is common in the elderly,is not only a manifestation of the deterioration of the body organs,but also a pathological response of nutritional status.Therefore,nutrition intervention for patients in the community has become a more popular trend in the treatment of diabetes.With the deepening of the reform of government management system,more and more social services are transferred to social organizations,and social work plays an increasingly important role in social service and management.However,literature review found that there were few studies on community nutrition intervention in elderly patients with diabetes during the intervention process,while social work intervention in this practice was almost non-existent.Choosing NH community as the place of investigation in Wuhan,this paper take semi-structured interview method to understand the basic situation in elderly patients with diabetes and the community.The potential resources were analyzed on the basis of the theory of ecological environment,and actual demand was found on the basis of the theory of self-efficacy,so we come to a conclusion that implementing nutrition intervention in the community for the elderly with diabetes was necessary and feasible conclusion.Then,with the goal of "improving self-efficacy and ensuring service sustainability",12 patients who volunteered to participate were recruited to provide group service for two months.The comparison of indexes before and after the service showed that the service could improve patients' blood glucose management ability and effectively improve their depression status.Finally,a community nutrition intervention service mode with "patients as the main body,community clinics as the leading,community neighborhood committee as the support,volunteers and family members as the supplement,and social work services throughout" was explored.
