At present, the problem of population aging has become a multi-disciplinary social problem, and long-term care for the elderly is one of the important components of the aging problem. For the elderly who are unable to take care of themselves for a long time or can’t take care of themselves at all,the local government will give some subsidies according to their disability degree, family economy and other conditions, and organizations from all walks of life and caring people will actively contribute. As a social worker who is people-oriented and provides voice and services for vulnerable groups,he has also joined the ranks of theoretical and practical research on long-term care services for the elderly. He has used the professional methods of social work science to intervene in the semi-disabled elderly and explore the best way to provide long-term care services. From the perspective of evidence-based social work, based on the community care theory and ecosystem theory, this paper will conduct a questionnaire survey and household interviews with 30 semi-disabled elderly people who have relocated from the poverty relief area in Y community, understand the family situation, care status and difficulties of the semi-disabled elderly in Y community, and summarize their common problems and needs. In combination with the way of case interview,we conducted in-depth interview and investigation on many of the elderly, and selected a semi-disabled elderly for case study.According to the results of a large number of previous investigations and evidence-based results,we completed the needs assessment work rigorously and scientifically. At the same time, following the evidence-based service concept, using the SPIDER model to construct the question of "what is the best path of long-term care service for the semi-disabled elderly",and using the systematic evaluation method of evidence-based social work science to retrieve and verify the evidence,it is concluded that the best path of social work intervention in long-term care service for the semi-disabled elderly is the result of case work.Based on this, accurately obtain the existing research evidence, and combine the actual experience and reality of the selected cases to match the best service plan, fully mobilize the strength of Y community, use community resources, and provide professional services for the service objects. So as to further promote the development of evidence-based social work in the theory and practice of long-term care services for the semi-disabled elderly, improve the quality of daily life of the semi-disabled elderly, and increase their sense of belonging and happiness. Finally, the evidence feedback work is carried out from the results of the service object satisfaction survey, the AUCL loneliness scale and the social support scale, the degree of service goal achievement, and the service provider self-assessment,to reflect and revise the research evidence in the service process. This paper summarizes the research conclusion of evidence-based practice in this study, that is,long-term care services for the semi-disabled elderly need to combine multiple forces to achieve the best results, affirms the importance of evidence-based practice in social work intervention, and emphasizes that the localization path should be taken in combination with reality. In addition,it combs the key points and difficulties, application limitations and shortcomings of evidence-based research, and puts forward relevant suggestions.
