Modern Literature Research On Acupuncture And Moxibustion Treatment Of Postherpetic Neuralgia And A Summary Of Mentors’ Experience

Objective:By collecting and sorting out modern literature on the treatment of postherpetic neuralgia by acupuncture and moxibustion,and collating statistics and data mining of the literature included in the database,this study summarized the disease characteristics and diagnosis and treatment rules of the treatment of postherpetic neuralgia by acupuncture and moxibustion,combined with the clinical experience of the treatment of the disease by the mentor to conduct a comprehensive analysis.In order to provide theoretical support for the clinical diagnosis and treatment of postherpetic neuralgia and the innovation of thinking.Methods:According to the search strategy formulated in this study,modern clinical literatures on acupuncture and moxibustion in the treatment of postherpetic neuralgia were searched from China Biomedical Literature Database(SinoMed),China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP database and Pubmed database.In addition,according to the inclusion and exclusion criteria,literature meeting the requirements was screened,a database was established,literature information was extracted,literature title,author,publication time,patient symptoms,syndrome,treatment methods,needle retention time,acupuncture acupoint prescription,moxibustion acupoint prescription and other information were input into the database.SPSS Modeler18.0 and IBM SPSS Statistics24 statistical analysis software were used to mine the data.At the same time,through the analysis of the mentor’s understanding of the etiology and pathogenesis of postherpetic neuralgia,the selection of treatment methods,combined with typical clinical cases to explore the mentor’ s treatment of postherpetic neuralgia.Results:(1)Through screening and sorting,a total of 326 articles meeting the entry criteria were included,17 clinical symptoms and 9 TCM syndrome types of postherpetic neuralgia were extracted,and 12 kinds of acupuncture and moxibustion treatment methods were used to obtain 309 prescriptions of acupuncture acupoints,involving 128 different acupoints,distributed in 5 parts of the human body and belonging to 14 meridians.Contains 9 specific acupoint classifications.(2)Frequency analysis results showed that the clinical symptoms of 17 postherpetic neuralgia were mainly local pain,the nature of pain was mostly stinging,the common TCM syndrome type was qi stagnation and blood stasis,acupuncture was the most commonly used treatment method,and the other common treatment methods were cupping,moxibustion and acupoint injection.Among the 128 acupoints used in the treatment of postherpetic neuralgia,the top 10 points used frequently were A Shi point,Jiaji point,Zusanli,Hegu,Taichong,Yanglingquan,Sanyinjiao,Quchi,Zhigou and Xuehai in order;According to the frequency of use of the acupoints,the locations of the acupoints were ranked from highest to lowest in the lower extremity,upper extremity,back,head and neck and chest and abdomen.Among the 14 meridians belonging to acupoints,the top 5 meridians used frequently were foot Taiyin Spleen meridian,foot Yangming stomach meridian,foot Shaoyang gallbladder meridian,hand Yangming large intestine meridian and foot Jueyin liver meridian in turn.Among the 128 acupoints,85 are specific acupoints,and the top three specific acupoints used frequently are the Wushou acupoints,the original acupoints and the Xihe acupoints.The commonly used acupoints of moxibustion for treating postherpetic neuralgia are Ashi point,Jiaji point,Zusanli point,Dazhui Point and Guanyuan point.It mainly selects acupoints on the back of the human body,mostly belonging to the governor’ s vein.(3)Association rule analysis results showed that the commonly used acupoint combinations were A Shi point-Jiaji point,Zusanli-Sanyinjiao-Jiaji point,Jiaji point-Zusanli-Ashi point,Hegu-Quchi and Yanglingquan.(4)Cluster analysis showed that acupoints could be divided into five categories:①Hegu,Quchi,Waiguan and Fengchi;②Neiguan,Baihui,Guanyuan;③Yin Lingquan,Xingjian,Zusanli,Sanyinjiao,Xuehai,Ganshu,Qimen,Geshu,Yanglingquan,Zhigou,Taichong;④Ashi point;⑤Jiaji point.Conclusion:(1)The main clinical symptoms of postherpetic neuralgia are local pain,the nature of pain is mostly stinging,and the syndrome type of traditional Chinese medicine is mainly Qi stagnation and blood stasis,(2)There are 12 acupuncture and moxibustion methods for the treatment of postherpetic neuralgia,among which acupuncture is the most commonly used method in clinical studies.(3)Acupuncture points are mostly located in the extremities of the post-herpetic neuralgia.The commonly used acupoints are ashi point,Jiaji point,Zusanli,Hegu and Taichong.Specific acupoints are used more frequently,among which Five Shu points,Yuanhe point and Xiahe point are the main acupoints.The acupoints on the spleen meridian of Foot Taiyin,the stomach meridian of foot Yangming,the gallbladder meridian of foot Shaoyang,the large intestine meridian of hand Yangming and the liver meridian of foot Jueyin were selected for treatment.(4)The most commonly used combination of acupoints in the treatment of postherpetic neuralgia is ashi point and Jiaji point.(5)Acupoint selection rules of acupuncture treatment of postherpetic neuralgia are mainly based on proximal acupoint selection and syndrome differentiation,supplemented by distal acupoint selection and meridian selection.The upper and lower acupoint matching method,the outer and inner meridian matching point method and the same name meridian matching point method are used.The instructor’s experience in treating postherpetic neuralgia:The instructor concluded that this disease should be the syndrome of deficiency of the original and deficiency of the solid,taking deficiency of healthy qi as the foundation and blood stasis and blockage of collateral as the standard.In the course of treatment,the basic principle of treatment,attention to syndrome differentiation of treatment,moxibustion,acupuncture,cupping method with a variety of methods.
