Clinical Study On Insomnia And Anxiety, TCM Constitution And Syndrome

[Objective]:Through evaluation of insomnia severe degree,anxious emotion,TCM syndrome types and consititution, analyzes the relationship between insomia and anxious emotion,and research the characters of TCM syndrome types and consititution of insomnia patients with (or without) anxiety,and analyse the correlation between TCM syndrome types and consititution. Thinking from the disease-emotion-constitution-TCM syndrome types four aspects and discussing comprehensively to deepen the understanding of insomnia,and provide the basis for clinical diagnosis and treatment.[Methods]:Investigate the 92 insomniac who came to the Neurology clinic or had a treatment in Dongfang Hospital of Beijing university of chinese medicine from July 2014 to January 2015. Using the basic information questionnaire to investigate the patients’gender,age.course of the disease,occupation,cultural degree,etc. Using the Pittsburgh Sleep quality Index (PSQI), Hamilton Anxiety Scale (HAMA),TCM four diagnostic information table, constitution of TCM scale to study the insomnia patients’severe degree,anxious emotion,TCM syndrome types and consititution. Using the descriptive analysis,correspondence analysis,linear correlation and chi-square test to deal with the collected information by SPSS 19.0 statistical analysis software[Result]:1. Most of 92 insomnia patients are middle aged and elderly people, the age mainly concentrated in the 51-60 years old, which proportioning 32%. The proportion of female patients is more than male. Patients with chronic insomnia in the proportion of 63%. Patients using sleep medication account for 37%. Companing with the increase of the educational level, the incidence of insomnia increased. Employed staff and employees who do many mental work are prone to insomnia. Among the survey population.extrovert proportion is higher than introvert’s.37.0% of the patients have anxiety.2. In the 92 patients, insufficiency of heart and spleen has the greatest number, the second is fire excess from yin deficiency,followed by liver depression transforming into fire.deficiency of heart qi and gallbladder qi, phlegm heat disturbing heart. Most female patients are fire excess from yin deficiency, while liver depression transforming into fire is most in male patients. Insomnia course and TCM syndrome types have certain ralations (P=0.039<0.05).In 58 patients with chronic insomnia, insufficiency of heart and spleen, fire excess from yin deficiency are most. In insomnia course less than 6 months of 34 patients, liver depression transforming into fire has the greatest number.3. In 92 insomnia patients, the number of people with qi depression constitution and blood stasis quality is largest,followed by qi deficiency, yin deficiency, yang deficiency, phlegm dampness,gentle quality, dampness-heate. It’s quite common to see qi depression in female patients,while male patients are mainly blood stasis quality. Different constitution types of HAMA scores are apparently different (P=0.005<0.05).Qi depression in HAMA score averages 16.79,which is the highest score.4. Insomnia of TCM syndrome types and constitution of traditional Chinese medicine has a certain relationship (P=0.004<0.05). Phlegm heat disturbing heart has closely relationship with phlegm dampness, insufficiency of heart and spleen is related with qi deficiency and yang deficiency, liver depression transforming into fire and dampness-heate relationship more closely, fire excess from yin deficiency is related to qi depression, blood stasis quality, Qi deficiency have a closely relationship with deficiency of heart qi and gallbladder qi. 5. People with different characters have a big difference in HAMA scores (P=0.013<0.05).PSQI scores and anxiety has certain relevance (P=0.031<0.05).Higher HAMA scores, patient’s PSQI score is higher, the more severe anxiety,the worse sleep quality。[Conclusion]:1. Women, high degree of culture, mental work, anxiety, middle aged and elderly people are more prone to suffer from insomnia. Anxiety of ansomnia patients with different personalities are different. There is obvious relationship between insomnia and anxiety. Sleep quality and anxiety severity significantly related.2. More cases of patients with chronic insomnia were insufficiency of heart and spleen and fire excess from yin deficiency. Most of patients whose insomnia course less than 6 months are liver depression transforming into fireo3. Insomnia patients constitution types in the majority with qi depression and blood stasis.
